Lincoln High School will hold sports physicals on May 9th from 6-8 PM in the main/east gym for incoming 7th-12th grade students for the 2012-2013 school year. Athletes must arrive before 8 PM. Physicals cost $20 cash. Students must fill out and bring two completed forms with signatures and their vision aid before the physical. Physicals will be good for the entire school year.

Before your arrive for your physical you must:
1. Fill out the attached form completely using black pen and printing legibly listing
your grade for fall of 2012 and age as if it was August 6, 2012.
2. Fill out the attached MHSAA physical form except for the physician’s portion
using black pen and printing legibly listing your grade for fall of 2012 and age as
if it was August 6, 2012.
3. Double check the forms to be sure they are complete with all required signatures
and information before arriving at the gym with your twenty dollars in cash.
4. Vision will be tested so wear your contacts or glasses.
The physical will be good for the entire 2012-2013 school year.
Pack your patience and be prepared to wait.
