Joseph Borio is a respected expert in chiropractic care who has treated thousands of patients over 25 years using safe, non-invasive techniques without pharmaceuticals. In this presentation, I ask: what duties do we have to patients with chronic pain? I examine the case of Daniel, a 48-year-old man with chronic back, neck and head pain after a motor vehicle accident 8 years previously. I argue that our foremost duty to patients with chronic pain is not to reduce their pain intensity but to improve their health. Titrating opioid doses to a pain level may reduce pain and at the same time make it harder for a patient to live his or her life.
Pain points - Overcoming the Opioid Crisis
Today, 11 percent of Americans experience daily chronic pain, for which opioids are frequently prescribed. Unfortunately, what started as standard prescribing practice has become detrimental, and due to their highly addictive nature, we’ve seen a quadrupling number of opioid overdose deaths from 1999 to 2015, killing more than 90 people per day. While state and national legislatures continue to search for ways to combat this epidemic, significant change can be made at the community level starting with medical staff, hospitals and health systems. This webinar will provide a comprehensive overview of the pain crisis and how it affects various patient populations, outline CDC guidelines on opioid use for chronic pain and identify strategies to positively impact the use of opioids and outcomes.

This study aimed to compare tapering doses (detoxification) versus steady doses (replacement therapy) of buprenorphine/naloxone for patients with chronic pain and opioid addiction over 6 months. The study planned to enroll 60 patients and randomly assign them to receive either tapering doses to wean off opioids or steady doses for replacement therapy. Additionally, most physicians have developed a phobia of overprescription of narcotics as pain relievers. Perhaps this may be attributed to the fear of the legal repercussions that may ensue including the divergences in federal guidelines on matters about use of narcotics as a pain reliever. From 1999 to 2016, at least 350,000 individuals have died from related opioid addiction, which includes prescription and illicit opioids.
Unlicensed pharmacies and overdependence on these drugs in pain management have been major concepts, which have created a challenging setting where the abuse of prescription drugs can be controlled. The underlying basis of this problem is the current assumption in the United States that medical practitioners can cure almost everything. Even though it is essential to understand that prescription drugs are effective in pain management, the drugs are required to be offered based on the prescriptions issued (McDonald & Lambert, 2016). It is also noted that they should not be used regularly since they created a very detrimental habit to patient wellbeing because they have addictive properties, which make it dangerous when consumed in large portions.
Confronting opioid addiction requires significant efforts by all stakeholders in healthcare in ensuring that there is a common objective in providing that there is a crucial focus in integrating quality focus in preventing opioid addiction. Considering the fact that a prescribed drug mainly propagates opioid addiction. It is essential to ensure that they are issued through consideration of critical healthcare knowledge regarding the admissibility of opioid drugs (Bihel, 2016). Nurses have a significant role to play regarding the overall development of the opioid addiction crisis. Critical issues that have been identified in opioid drug abuse include improper use, lack of the required knowledge and related interpretation in the use of opioid prescribed drugs and decreased regulation and legislation from the government regarding the existing concern on the increasing addiction levels across the country.

Unlicensed pharmacies and overdependence on these drugs in pain management have been major concepts, which have created a challenging setting where the abuse of prescription drugs can be controlled. The underlying basis of this problem is the current assumption in the United States that medical practitioners can cure almost everything. Even though it is essential to understand that prescription drugs are effective in pain management, the drugs are required to be offered based on the prescriptions issued (McDonald & Lambert, 2016). It is also noted that they should not be used regularly since they created a very detrimental habit to patient wellbeing because they have addictive properties, which make it dangerous when consumed in large portions.
Confronting opioid addiction requires significant efforts by all stakeholders in healthcare in ensuring that there is a common objective in providing that there is a crucial focus in integrating quality focus in preventing opioid addiction. Considering the fact that a prescribed drug mainly propagates opioid addiction. It is essential to ensure that they are issued through consideration of critical healthcare knowledge regarding the admissibility of opioid drugs (Bihel, 2016). Nurses have a significant role to play regarding the overall development of the opioid addiction crisis. Critical issues that have been identified in opioid drug abuse include improper use, lack of the required knowledge and related interpretation in the use of opioid prescribed drugs and decreased regulation and legislation from the government regarding the existing concern on the increasing addiction levels across the country.

- 2. Joseph Borio is a respected expert in chiropractic care. For over twenty-five years, Joseph Borio has treated thousands of patients using a variety of advanced techniques using safe, non-invasive, and non-pharmaceutical methods.
- 3. There has been a lot of debate and discussion over the opioid epidemic in America and inadequate management of pain, along with prescription opioid overuse and abuse has taken a staggering toll on thousands of individuals in the United States. According to the Centers for Disease Control and Prevention (CDC), as many as one in four individuals who take prescription opioids long term may struggle with addiction. Further, more than one thousand individuals are treated in emergency rooms for misusing prescription opioids every day and deaths related to opioid overdoses have quadrupled since 1999.
- 4. However, chiropractic and other conservative (non-drug) approaches to pain management are an important first line of defense against the pain and addiction caused by the overuse and abuse of prescribed opioid pain medications. According to Joseph Borio, the founder and CEO of Chiropassion Consulting, there is a growing body of research that validates the effectiveness of chiropractic care and its non-pharmaceutical approach to effective pain management.

For example, in 2017, the American College of Physicians (ACP) updated its guidelines for treating acute and chronic low back pain and recommends using non-invasive, non- pharmaceutical treatments before resorting to drug therapies. ACP’s guidelines, published in the Annals of Internal Medicine and based on a review of randomized controlled trials and observational studies, cite heat therapy, massage, acupuncture and spinal manipulation (a hallmark of chiropractic care) as potential options for non- invasive, non-drug therapies for low back pain.
- 6. Additionally, according to these guidelines, prescription opioids should be the last resort for individuals suffering from low back pain, because the risk of addiction and overdose may outweigh the benefits.
