The following is for information purposes to assist you if you have been a victim of a crime. Please
read and understand each agency’s privacy and security policies before using their services.
Report identity theft and fraud to the following:
1- Each of the three national consumer reporting agencies. Ask each agency to place a fraud alert on
your credit report, and send you a copy of your credit file.
2- The fraud department at each creditor, bank, or utility/service that provided the identity thief with
unauthorized credit, goods or services.
3- The Federal Trade Commission, which maintains the Identity Theft Data Clearinghouse, the
federal government’s identity theft complaint database and provides information to identity theft

To report fraudulent use of your checks:
Contact your bank if you have checks that are stolen or if bank accounts have been opened
fraudulently. Call the check verification companies that monitor approved check transaction activity.
