We understand and are dedicated to meet your needs for visit adherence and patient retention. Our company has been extremely successful in managing worldwide patient transportation for the past 10 years by working closely with CROs, Pharmaceutical companies and clinical trial sites to coordinate transportation during clinical trials. Introducing CPT SERVICES Patient Transportation ♦ Worldwide patient transportation during clinical trials ♦ We secure and coordinate the entire trip to and from the patient's home to the clinical trials' sites (including but not limited to taxis, car rentals, ambulances, hotels and flights) ♦ Fixed cost for the coordination of transportation ♦ The transportation can be as simple as a taxi ride or as complex as an ambulance picking up the patient, taking them to the airport, booking a hotel, reimbursing their dinner, taking them to the site and returning them to their destination. - 3. 2 Gatewood |Irvine | CA | 92604 | USA T: (949) 433-7838 www.EventsByAgora.com CPT An Agora Group Company Overview-July-2014 Clinical Patient Transportation CPT Pricing Structure Profiling costs ($100 per patient “one time fee”): Each patient is profiled where we determine all information, First name, last name, address, phone number, caregivers, preferred communication method etc. We provide them with a project number (this code is normally provided by yourself to carry through the project). Ie His initials-project_number_site_number. • This code is used for our billing (ie Invoice 1690-PJS-IT-Aug-20-2011). • This is study 1690 – Patients initials - country – date of visit. 800 Numbers: North America $40 a month – Europe $100 a month + communication fees when applicable. Fees: Our fees vary from $50 to $200 based on the complexity of the transportation. Flights: We always book flights at the lowest possible fare and are non-refundable with a change fee in case of cancellation or changes. Ground Transportation: Sedan, SUV and ambulances are available Reimbursement and advances: Certain of our patients do not have funds to pay for gasoline or hotels and we “front” them the money on a per diem basis or against receipts. However due to some therapeutic conditions dealing with elder people we do not always request receipts. Partner Fees “Europe”: Are pass through as all other fees such as hotel costs, flights, meals etc … Cancellations: Typically we try and recuperate all the credits “vendor dependent” and apply to next trip and do not apply a fee for cancellations. Billing/payment terms: We bill you normally send you an invoice 1 to 10 days after visit with a 15 day term. (Contracts will vary and deposits may be requested). Deposit/Retainer: Based on the study size and number of cash-advances a retainer may be requested. Guidelines: We request guidelines to be provided to us in regards to costs, however fluctuations in prices are seasonal and based on our timeline to book tickets. We will closely monitor costs and call you for approval in the event pricing is considerably higher than the norm
