WHAT are we?Citizens Care and Rehabilitation Center (Citizens):170 bed Skilled Nursing Facility and Rehabilitation Center located on Rosemont Ave, across from Fort DetrickResident  skilled nursing & custodial care  paid for by Medicare, Medicaid, private insurances, and private pay fundsApproximately 220 staff members provide services, and include: Nurses, Aides, ancillary, and administrative staff. Also employs a Medical Director (physician), rehabilitation therapists, psychologists and other professionals.Received a four out of five Star rating by Centers for Medicare & Medicaid Services for the past two years
WHAT are we?Montevue Assisted Living (Montevue):All private room Assisted Living Facility located adjacent to Citizens, with an entranceway off of Montevue LaneResident care paid for with private pay funds and County subsidies when necessaryResidents can participate in Medicare B, Medicaid, and other private insurance programsApproximately  40 staff members care for our 60 residents, along with a Medical Director, rehabilitation staff, and other professionals.
How are we the SAME?Both facilities are owned and operated by Frederick County Government, governed by a volunteer Board of Trustees and the County CommissionersBoth are licensed by the Maryland Department of Health & Mental Hygiene and receive annual quality surveysBoth provide family-oriented care with the best interest of the residents in mind.Both provide end of life care when needed
How are we DIFFERENT?Citizens:Skilled Nursing Facility, or what used to be called a “Nursing Home.”  Must meet specific clinical criteria in order to receive insurance coverage for an admission. Most commonly receive admissions from hospitals, other nursing homes, or assisted living facilities.Montevue:Assisted Living Facility, allowing people who may not be able to function ideally in a home setting any longer to live in a home-like atmosphere with some assistance from professional and ancillary staff members.Residents can perform many of their Activities of Daily Living (ADL’s) independently, but may need varying levels of assistance.
Some HistoryCitizens:  Built in 1976 to house 120 residents , “D” Wing completed in 1981, adding 50 beds, a dining room, activities room, and indoor gardens to meet expanded need and add home-like touches.Montevue:  built in 1870 as a solution, at the time, to help care for Frederick County’s frail elderly. Moved across Montevue Lane in 1987 to newly constructed facility with 60 resident rooms, all private.

Where and Why New Facilities?Brand-new Assisted Living and Skilled Nursing facilities on the same site - just across the parking lot from our current location, on Rosemont Avenue across from Fort DetrickFor Montevue, all residents will live in graciously appointed private efficiencies, reaturing private bathrooms and “kitchenettes”For Citizens, gone will be triple person “wards.” Instead, all will enjoy increased privacy in either private or semi-private rooms with full walls separating living spaces (residents in doubles will still share a bathroom).We will be able to better meet the needs of our residents with state of the art design & programming
When Do We Move In?The new facilities will open at the end of 2011 or beginning of 2012, with open house tours beginning sometime before we move our residents into their new home
What are the New FacilitiesLike?Citizens will feature:74 attractive Private Rooms, 48 Semi Private rooms with private spaces for beds and private bathrooms - all with furniture, electric beds, call bell system and curtains Several comfortably large recreation spaces/rooms for the many activities, as well as areas for residents and families to meet publicly and privatelyGraciously sized and appointed dining rooms on every resident neighborhood, featuring seated service & individual menusCatered dining timed around when each resident wishes to eatBeauty Salon, Dental & Podiatry Suite, and Gift Shop	Special lifts for bariatric residentsOxygen piped into the rooms – no more bulky tanks or unsightly equipment required!
