This document is a Competent Person Certificate issued by the Director of Industrial Safety and Health of Gujarat to Shri Chhatrapalsinh B. the powers conferred on me under Section 2 (ca) of the Factories Act and the rules made there
undir hereby recognize Shri Chhatrapalsinh B. Gohil residing at 45, New People's Society,
Subhash Nagar, Bhavnagar-364 001 to be a competent person for the purpose of calrying out
tests, examinations, inspections and certifications under following sectior/s and the rules made
there under in a factory located in Gujarat State.

This certiJicate is issued subjectto the conditions stipulated hereunder:-
1) Tests, examinations and inspections shall be carried out in accordance with the
provisions of the Act and the rules made there under.
2) Tests, examinations and inspections shall be carried out under the direct supervision of
the competent person or by a person so authorbed by an institution recognized to be q
competent person.
3) The certificate of competency issued in favour of a person shall stand cancelled if the
person declared competent leayes the organbation mentioned in his application.
4) The institution recognized as a competent person shall lceep the Director, Industrial
Safep & Heakh informed of the names, designations and qualifications of the persons
authorized by it to carry out tests, examinations and inspections.
, The copies of certificates of examinhtions /. tests isswed by you after due examinations /
tests are to be marked to the Inspector of Factories concerned within seven days in all
cases where defects are noticed and repairs or alternations are suggested on its use.
6) The record of the work done by the competent person should be maintained in the Log
Book, incorporating therein the details regarding work done, obseruations rna.de,
directives give4etc.
7) Certificate No. and validity period should invariably recorded by you in all the copies
of certificates of examinations / tests signed and issued by you.
8) Every exarnination / test shall conform to the relevant standard, if it is not specified in
the Act or rules made there under.
9) Facilities and equipments to be used by you should be mqintained and kept in a good
and fficient working conditions at all times and calibratedfrom time to time.
10) Application for renaual of Competent Person Certificate shall be made at least two
months before the date of expiry of the certificqte to the Director, trndustrial Safety &
Health with Log bookfor the period afprevious nine months in cqse offirst renewal and
twelve months in case af second and subsequent renewal.
l1) Save as provided in Rule 6l(9)(a) of section 31, in no case conditional certificate shall
be issued.
