This document reports on a rare case of a glandular odontogenic cyst (GOC) that was misdiagnosed as a dentigerous cyst. A 38-year-old woman presented with swelling in her palate. Radiographs and CT scans revealed a unilocular radiolucency surrounding the crown of an impacted canine tooth. The cyst was surgically removed and histopathological examination confirmed it was a GOC. GOCs are rare developmental cysts that can be difficult to diagnose due to non-specific clinical and radiological features resembling other odontogenic lesions. Long-term follow-up is important given the potential for recurrence of GOCs.
This document discusses sepsis, septicemia, and multiple organ dysfunction syndrome (MODS). It defines related terms like bacteremia, systemic inflammatory response syndrome (SIRS), sepsis, severe sepsis, and septic shock. It discusses the magnitude of the problem globally and in India. It covers the pathophysiology and most common sources of infection. It also summarizes the Surviving Sepsis Campaign guidelines for management of sepsis and septic shock. Finally, it provides an overview of recent advances and controversies in the field and lists various biomarkers that can be used for diagnosis, prognosis, and stratification of sepsis severity.
1. The study examines the reduction in size of keratocystic odontogenic tumors (KCOTs) in the mandible via marsupialization and 3D CT scans over time.
2. Fifteen patients with KCOTs underwent marsupialization surgery, and their 42 serial CT scans were analyzed to measure tumor diameter and volume changes.
3. The results show that KCOTs reduced equally towards the marsupialization window and that volume reduction per day followed the formula Vr = -0.0029 × V, indicating volume is reduced by half over 239 days of marsupialization. This allows prediction of tumor size changes over time.
This study evaluated 14 cases of autogenous maxillary canine transplantation in young patients. Impacted or partially erupted maxillary canines were extracted and immediately transplanted to prepared recipient sites. Two years later, 12 cases showed normal mobility and lamina dura formation on radiographs, indicating successful transplantation. Two cases showed inflammatory resorption, increased mobility, and widening of the periodontal space, indicating unsuccessful transplantation. Autotransplantation of impacted or partially erupted maxillary canines can be a viable alternative to other treatments like orthodontics or dental implants when patient selection criteria are met and proper surgical technique is followed.
This study compared two radiographic techniques - vertical parallax and magnification - for localizing impacted maxillary canines. Six examiners used each technique to predict the position of 133 impacted canines based on panoramic and occlusal radiographs. Their predictions were compared to the actual positions identified during surgery. Localization was more successful using vertical parallax (76% correct) than magnification (66% correct), though the difference was not significant. Both techniques correctly identified almost 90% of palatally impacted canines, but less than half of buccally impacted canines could be detected using parallax and only 10% using magnification. The study suggests vertical parallax may be slightly better but neither technique is clearly superior, and additional views are
The document describes a new bus service called The Renaissance that aims to provide the privacy of individual train compartments at a lower cost and environmental impact than cars or regular buses. The Renaissance buses would have removable dividers to create private seating areas for individuals and families, as well as amenities like TVs. Initial routes would connect Boston and New York, and the service seeks to offer greater comfort, privacy and ability to work than traditional bus services through its compartmentalized design.

This case report describes two cases of glandular odontogenic cyst (GOC). The first case was a 42-year-old female presenting with a swelling in the mandible extending from teeth 19 to 29. Radiographs showed a multilocular radiolucency. The second case was a 21-year-old male with a large swelling in the mandible. Both lesions were diagnosed as GOC based on histopathological examination showing characteristics like cuboidal and ciliated epithelial cells and PAS-positive mucous cells. GOC is a rare cyst that can be difficult to diagnose and has a potential for recurrence due to its biological behavior and treatment approach.

This case report describes a rare case of an inflammatory dentigerous cyst associated with an unerupted mandibular first premolar tooth in a 9-year-old male patient. Dentigerous cysts typically involve the mandibular third molar and are uncommon in the first decade of life. Imaging revealed a well-defined radiolucent lesion associated with the crown of the unerupted first premolar. Histopathological examination after surgical removal showed a cyst lining of non-keratinized stratified squamous epithelium with inflammation in the connective tissue wall, consistent with an inflammatory dentigerous cyst. Complete removal of the cyst and associated tooth is important to prevent potential complications. This case highlights

This study evaluated the effects of Chinese herbs on volatile sulfur compounds (VSCs) associated with halitosis. 40 herbs were tested, with 14 found to reduce VSCs by over 50%. 10 herbs showed over 70% inhibition of bacterial growth. A dentifrice containing chrysanthemum flower, one of the most effective herbs, significantly reduced VSCs compared to controls and other commercial dentifrices, demonstrating potential for treating halitosis.
This document reviews the reclassification of the odontogenic keratocyst (OKC) as a keratocystic odontogenic tumour (KCOT) based on its aggressive behavior. It summarizes a case series of 27 KCOTs treated at one clinic which had a 29% recurrence rate, with all recurrences occurring within 2 years. Most lesions were between 0-15 cm2 in size. The reclassification underscores the need for aggressive treatment such as enucleation with Carnoy's solution or marsupialization followed by cystectomy to reduce recurrence rates. Future molecular treatment may provide less invasive options.
Anaesthetic Considerations in Neuromuscular diseaseHemant Ojha
This document discusses anaesthetic considerations for patients with neuromuscular disorders. It begins with an introduction to neuromuscular disorders and their classification. General considerations for pre-operative, peri-operative, and post-operative management are outlined. Specific guidance is provided for various disorders like Duchenne Muscular Dystrophy, Guillain-Barré syndrome, and Myasthenia Gravis. Key points are that depolarizing neuromuscular blocking agents should generally be avoided due to risk of hyperkalemia. Non-depolarizing agents may have prolonged effects. Careful monitoring is needed due to potential respiratory and cardiac involvement. Thorough pre-operative evaluation and planning of anaesthetic approach is important for
Sequential Organ Failure Assessment (SOFA) ScoreHemant Ojha
Sequential Organ Failure Assessment (SOFA) score is a severity of illness scoring system used in critical care units to assess how organ function is affected over time. It measures function of the lungs, liver, kidneys, coagulation, heart, and nervous system. Higher scores indicate more severe organ dysfunction. Studies have validated SOFA for predicting mortality, with scores increasing over 48 hours associated with 50% mortality and decreasing scores 27% mortality. A maximum SOFA score over 15 is associated with 90% mortality. The SOFA score provides an objective measure of organ dysfunction that can be used for assessing prognosis and allocating resources in intensive care units.
