A 42-year-old man presented with a slowly growing anterior neck swelling for 2 years that mimicked a goiter. On examination, the swelling was soft, not compressible, and more prominent with muscle contraction. An ultrasound revealed a hypoechoic neck mass while the thyroid was normal. The final diagnosis was an anterior neck lipoma, which is a very rare condition.
