- The sheer volume of events (hospitalizations, surgeries, office visit) spread out over a large health system makes analysis a challenge. Sequence: It is often the "handoffs" between providers where mistakes happen or miscommunication effects the desired outcome. Timing: Long wait times for diagnostic tests and slow follow up of test results can lead to delayed diagnosis or even disease progression.
