1. Generation Putnam 3 on 3 Basketball Tournament
Release and Waiver of Liability
In consideration of being permitted to engage in the Generation Putnam 3 on 3
Basketball Tournament to be held on June 26, 2010 (the "Tournament") at Hurricane High
School and other locations (the "premises"), I (the “Participant”) hereby release, waive and
discharge Generation Putnam, the Putnam County Chamber of Commerce, the Putnam
County Board of Education, their respective directors, officers, trustees, employees, volunteers,
sponsors, promoters, and agents from any and all liability to me, my next of kin, heirs, and
personal representatives arising out of or in any way relating to my participation in any activities
during or related to the Tournament.
(Please initial each paragraph in the space provided. Legal guardians should initial if signing
for a minor participant. )
______ I understand that I will be engaging in activities which involve risk of serious injury,
including permanent disability and death, that severe social and economic losses might result not
only from my actions, inactions or negligence, but also from the intentional, reckless, or
negligent actions or inactions of others, the rules of play, or the condition of the premises, or of
any equipment used, and that there may be other risks not known or not reasonably foreseeable
at this time. I am voluntarily engaging in activities during or related to the Tournament knowing
the risks and present conditions and acknowledge the potential for hazardous or dangerous
conditions that may suddenly arise.
______ I assume all of the foregoing risks and accept personal responsibility for any damages
following any such injury, permanent disability or death and hereby agree to defend, indemnify
and hold Generation Putnam, the Putnam County Chamber of Commerce, and the Putnam
County Board of Education harmless from and against any liabilities, causes of action, suits,
claims or expenses (including reasonable attorneys’ fees) resulting or arising from my participation
in activities during or related to the Tournament.

2. ______ In signing this Release and Waiver of Liability, I hereby acknowledge and represent
that I am over eighteen (18) years of age, of sound mind, and competent to execute this Release
and Waiver of Liability. If the Participant is under 18 years of age, the minor’s legal guardian
must sign this release form on behalf of the minor and write the minor’s name, age and the word
“minor” next to the legal guardian’s name.
I ACKNOWLEDGE THAT I HAVE READ THIS RELEASE AND WAIVER OF
LIABILITY CAREFULLY AND FULLY UNDERSTAND THAT BY SIGNING IT I AM
WAIVING ANY RIGHT THAT I MAY HAVE TO BRING A LEGAL ACTION OR
ASSERT A CLAIM AGAINST GENERATION PUTNAM, THE PUTNAM COUNTY
CHAMBER OF COMMERCE, THE PUTNAM COUNTY BOARD OF EDUCATION,
THEIR RESPECTIVE DIRECTORS, OFFICERS, TRUSTEES, EMPLOYEES,
VOLUNTEERS, SPONSORS, PROMOTERS AND AGENTS.
