Waiver for participating in the Brandon Invitational Tournament 2012

1. First Annual Invitational Karate Competition

Agreement and Release
In consideration of my being permitted to compete in the First Annual Invitational Karate Competition to be held
in Brandon, Mb. on October 13th, 2012
I, ____________________________________________________, agree on behalf of myself and my heirs,
(Competitor’s name)
Successors and assigns, as follows:
1. I release Budo International Karate Diagaku of Manitoba, JKA-IKD Brandon and all of its officers, directors,
stockholders, employees, instructors and representatives from all liability and claims which I may have
against them for any injury or bodily harm, resulting from my participating in the Competition or in travelling
to, from or about the Competition, whether or not such claim be based upon negligence of such persons or
entity.
2. I accept full financial obligations incurred for medical, hospitalization and other expenses related to injury or
bodily harm, resulting from my participating in the Competition or in travelling to, from or about the
Competition.
3. I release and hold harmless Budo International Karate Diagaku of Manitoba, JKA-IKD Brandon and all of its
officers, directors, stockholders, employees, instructors and representatives from claims or liability for those
expenses described in paragraph 2 hereof.
4. I authorize the following named person to act in any matter requiring my consent, including their retaining
on my behalf medical attention and hospitalization and related services, should I be unable to give such
consent.

5. I consent to the use, free of any compensation to me, by Budo International Karate Diagaku of Manitoba, of
any photographs taken of me in any connection with the Competition.
6. I am 18 years of age or older. I understand and fully recognize the risk of physical injury or bodily harm in
participating in a karate tournament and with such knowledge and recognition; I hereby assume all risk of
such injuries or death resulting from my participation in the Competition. If I bring legal action in violation of
the terms of this Agreement and Release, I agree to pay the reasonable attorney fees and costs of the
party against whom action is brought.
