Recommended Organizational Structure and Staffing Complement for the LGUs

● Guides in the Staffing of an Organization
a. Reduce functions/programs into activities/tasks to identify staffing requirements.
b. Determine skills/competencies required to perform the functions/activities/tasks.
c. Match activities/tasks with positions in the Index of Occupational Services, Position Titles and Salary Grades (IOS) for the
National Government to determine the appropriate position title. Whenever possible, preference should be given to generic
over specialist positions, for flexibility.
d. Determine the number of positions required based on workload.
e. Apart from the unit head, allow one-of-a-kind higher or senior level position and provide at least one entrance level position.
f. As much as possible, limit the provision of assistants.
g. Observe an optimal ratio between technical and administrative positions.
● Identify functions (core and non-core) that could be effectively outsourced/contracted out; provided that
accountability mechanisms are in place.
● All staffing modifications involving reclassification, conversion, and creation of positions shall be in
accordance with the IOS for National Government.

● Manage the coordination of health service delivery
operations of PCPNs, hospitals and other health facilities
● Oversee the overall integration of the health systems and
implementation of public health programs, including referral

and disaster risk reduction and management for health.
Health System Support Division
● Manages:
○ health financing (planning and budgeting)
○ health information system
○ procurement and supply chain for health products and
services
○ local health regulation
○ health human resource development

Administrative Unit
● Render administrative and logistics assistance to the other
units within the PHO.
● Coordinates with other offices within the Provincial
Government and its component local government units;
● Manages internal logistics and property management;
● Manages facility, equipment and motor vehicle
maintenance, among others.

health systems meet the minimum quality standards set by
the DOH, PhilHealth and other stakeholders, and satisfy the
needs and expectations of the clients.

shall provide both population and individual-based health services
● Provide leadership in patient navigation and coordination in a network and
follows the standards set by the DOH

  • 2. Department of Health, Philippines ● Guides in the Staffing of an Organization a. Reduce functions/programs into activities/tasks to identify staffing requirements. b. Determine skills/competencies required to perform the functions/activities/tasks. c. Match activities/tasks with positions in the Index of Occupational Services, Position Titles and Salary Grades (IOS) for the National Government to determine the appropriate position title. Whenever possible, preference should be given to generic over specialist positions, for flexibility. d. Determine the number of positions required based on workload. e. Apart from the unit head, allow one-of-a-kind higher or senior level position and provide at least one entrance level position. f. As much as possible, limit the provision of assistants. g. Observe an optimal ratio between technical and administrative positions. ● Identify functions (core and non-core) that could be effectively outsourced/contracted out; provided that accountability mechanisms are in place. ● All staffing modifications involving reclassification, conversion, and creation of positions shall be in accordance with the IOS for National Government.   1. Good day! The following slides will present to you the Recommended Organizational Structure and Staffing Complement for the LGUs in light of the Devolution Transition Plan. This incorporates the proposals from technical offices of the Department of Health, as well as directions and agreements from the meeting between the Bureau of Local Health Systems and Development Health Facility Development Bureau Health Policy Development and Planning Bureau Health Human Resource Development Bureau Personnel Administration Division of the Administrative Service.
  2. The Staffing review was guided by the DBM-CSC Government Rationalization Program Organization and Staffing Standards and Guidelines as shown in your screen. Specifically, the staffing requirements was developed by: Identifying the functions expected of the organization, and Identifying positions titles that are more generic than specific, and based on the latest INDEX of OCCUPATIONAL SERVICES, OCCUPATIONAL GROUPS, CLASSES and SALARY GRADES (105) CY 2018 Edition (IOS released by the DBM in 2018). There is also a hierarchy of positions, reflecting one head of office/unit, one senior level position, and at least one entry level position. We note that the existing structure/HR complement in the different levels of the Local Government Units does not follow a prescribed organizational structure and staffing. Hence, the organizational structure that you will see is a proposed ideal/minimum staffing complement which should be enough to perform the expected functions of the organization. The LCEs may add more, depending on the needs of their LGU and local health system, as provided for in the Local Government Code of 1991.

  4. In this slide, you will see the recommended organizational structure for the Office of the Provincial Health Officer. This presentation will focus on the staffing complement for the: Office of the PHO as the technical secretariat of the Provincial Health Board. Health Service Delivery Division which will manage the health service delivery operations of PCPNs, hospitals, and other health facilities, and oversee the implementation of public health programs including health promotion, epidemiologic surveillance and disaster risk reduction and management for health. Health System Support Division which will manage the health financing (planning and budgeting), health information system, procurement and supply chain for health products and services, local health regulation, health human resource development, and performance monitoring, among others, in close coordination with the concerned offices of the provincial/ city government. Administrative unit will render administrative and logistics assistance to the other units within the PHO.
  5. This slide will show you the recommended minimum staffing complement for the office of the provincial health officer. The office of the PHO will be headed by a PHO II as the head of office. He/She is mainly responsible for the technical integration and supervision of the P/CHWS. The Divisions will be headed by a Division Chief. For the HSDD, this will be headed by the Asst. PHO (AO 2020-0021 Sec. VII.B.2.b.i.1) For HSSD, this will be headed by a CHPO. The Division Chief will be assisted by an Assistant Division Chief. All units under each division will have: 1 unit head 2-3 technical staffs, depending on the extent and complexity of the expected functions of that unit; and 1 administrative assistant or encoder to support the unit. This will be the structure that you will see on the next slides which is composed of the units under a division. On the other hand, the administrative unit under the PHO which will be headed by an Administrative Officer V, IV, II and Administrative Assistant III to support the entire unit. This structure will be mirrored in the City and Municipal Health Offices, as you will see in the succeeding slides.

Under the PHO is the Health System Delivery Division which is composed of four (4) units for Health Promotion, Epidemiology and Surveillance, Disaster Risk Reduction and Management, and Public Health Program Management. The functions of these units are reflected in the gray boxes on your screen. As mentioned earlier, each unit have a unit head, technical staff, and an administrative support. For example, looking at Health Promotion Unit, this will be headed by a HEPO III, 2 HEPO 2s and 1 Administrative Assistant III. These were streamlined structure from the original proposal of the technical offices. To highlight, the Provincial Epidemiology and Surveillance Unit was originally composed of 13 staffing complement, however in this structure we proposing to have only 5 as the minimum staffing complement. This is in alignment with RA 11332 IRR Rule VII. Sec.2 that at there should be least 1 DSO for a PESU. The Disaster Risk Management Unit which was originally proposed to have 11 was streamlined to have 5 staff. While Public Health Program Management Unit which was originally proposed with a 23 staffing complement was also streamlined to 6. The next slide will show you the other division of the PHO. The Health System Support Division, headed by a Chief Health Program Officer and assisted by a Supervising Health Program Officer, will have five (5) units: We propose to merge the Planning Unit with the Quality and Performance Monitoring Unit which will have 6 staff to manage the health planning process, and to ensure that performance standards are consistent across health facilities. The Health Information Systems Unit will have 4 staff which will be managing the information and communication technology development, management and interoperability of information systems and infrastructure, and health information resources for the PHO. The Supply Chain Management Unit will also have four 4 staff to handle demand planning and forecasting, inventory, supply transport and warehousing, and monitoring of storage and disposal. While the HRH Management and Development Unit will have 4 to manage HRH management and HRH development.
  8. Next to be presented is the recommended organizational structure and staffing for the Office of the City Health Officer in Highly Urbanized Cities (HUC) or Independent Component Cities (ICC).
  9. This organizational structure for the City Health Office mirrors the set-up of the Provincial Health Offices. However, it will show you that the CHO will also be managing urban health centers. The Urban Health Centers will be the first contact of primary care services in the city. The next slide will show you the: Office of the CHO as the technical secretariat of the City Health Board.   12. Similarly, we are recommending the merging of these two (2) small units which are the Planning unit and Quality and Performance Monitoring Unit. The same thing with the PHO it will have these 3 more units under the health systems support division. However, the next slide will show you an additional unit under the City Health Office which is the Urban Health Center on a primary care facility in the city.

Based on the Primary Care Facility Standards as disseminated through DC 2020-0176, this slide shows you that under the Urban Health Center there will be a Health Service Delivery Unit as well as the Health Systems Support Unit and also one (1) or several Barangay Health Stations. The staffing complement under the Health Service Delivery Unit are as follows: Headed by a Medical Officer IV Medical Officer III will be computed based on the 20K population Nurse II which also be based on a 10k population As well as the Midwife and Sanitation Inspector based on the population ratio Lastly, an HPO II to manage disease surveillance will be under this unit, as provided in RA 11332 and its IRR. The composition of this unit will be a dynamic one depending on the population ratio which the UHC serves. On the other hand, the Health Systems Support Unit will be: headed by a Supervising Health Program Officer And will also be supported by a Information System Analyst to handle the health information systems of the center As well as the Administrative Officer III to manage the systems support services such as supply chain and HR management The Barangay Health Stations will be manned by a Nurse and a Midwife and will be supported by a Barangay Health Worker or a Barangay Health Aide, Barangay Nutrition Scholar, Administrative Assistant , a Utility worker and a driver. At this point, I would like to highlight that this structure proposes for Barangay Health Aide positions (as listed under the Index of Occupational Services issued by DBM) in Primary Care Facilities for hiring of qualified health volunteers (i.e. Barangay Health Worker and Barangay Nutrition Scholar)
  14. Next to be presented is the recommended organizational structure and staffing for the Office of the City Health Officer in Highly Urbanized Cities (HUC) or Independent Component Cities (ICC).
  15. This slide will show you an almost similar structure for the Municipal Health Office or a City Health Office in a Municipality or Component City. For this organization, there can be 2 scenarios: 1st scenario: The Municipal/City Health Office is one and the same with the Rural Health Unit/Urban Health Center and has no hospital owned and managed by the component LGU 2nd Scenario: There are several Rural Health Units/ Urban Health Centers under the Municipal/ City Health Officer and has a hospital owned and managed by the component LGU
  16. In this slide you will see the staffing complements listed in the right table under the Municipal/City Health Office.
  17. Again, based on the Primary Care Facility Standards, the RHU or Urban Health Center have a Health Service Delivery Unit as well as the Health Systems Support Unit and also one (1) or several Barangay Health Stations. The minimum staffing complement under these units nd Barangay Health Station is similar to the one discussed earlier, and the composition of this unit will be a dynamic one depending on the population ratio which the UHC serves. The difference, however, is for Scenario 1, when the MHO is one and the same with the RHU. The MO V or the CHO II can also function as the head of the RHU and the health service delivery unit altogether. For Scenario 2, if there are several RHUs or Health Centers in the municipalities, the LGU should have an RHP to manage the RHU or UHC, and should have an MO IV as a unit head of the Health Service Delivery Unit. Once the presented organizational structures and staffing are approved by our principals, HHRDB and PAD will update the corresponding QS, Competencies and job descriptions.
