Liberty Medicare is a full service, licensed
independent insurance agency specializing in
various Medicare plans for people above age 65 or on
disability, Individual Health Insurance plans for
people under the age of 65, and Long-Term Care
plans.

People age 65 or older.
People under the age of 65 with

failure requiring dialysis or a

Medicare Supplement Plan N PA

Medicare Planes
Medicare Part A – Hospital Insurance:
Medicare Part A is the Hospital Insurance
portion of Original Medicare. Original
Medicare allows you to use any doctor,
specialist, or hospital that accepts Medicare.
You do not need a referral.
 You are paying a separate amount for each
service, also called fee for service.
Hospice care services
Inpatient care in a skilled nursing facility,
however, this doesn’t include custodial or
long term care.
Medicare Supplement Plan F PA

Medicare Plane A Benefits
Medicare Part A coinsurance, plus 365 additional
days after Medicare benefits end.
The first 3 pints of blood.
Hospice Care coinsurance/copayment.
Not Include
Foreign Travel Emergency – Medicare coverage
outside of the US.
Part A Deductible: $1,184 per benefit period in
2013.

Medicare Plane C
Medigap Plan C, also known as Medicare
Supplement Plan C, is the second most
comprehensive plan (after Medigap Plan F). It
covers all Medicare ‘gaps’ with the exception of
Medicare Part B Excess Charges.
Not all Medicare Supplement insurance
companies offer Plan C. Some companies offer
only one of the two most comprehensive Medigap
plans: Plan F or Plan C.

Plan C benefits include:
Hospital Costs: Medicare Part A coinsurance and
365 additional days after Medicare benefits end.
Medical Expenses: Medicare Part B coinsurance
(typically 20% of expenses that are Medicare
approved) or copayment.
Blood Coverage: The first 3 pints of blood per
each calendar year.
Foreign Travel Emergency (up to Plan Limit),
consisting of Medicare coverage outside of the US.

Medicare Part A – Hospital Insurance
Medicare Part A is the Hospital Insurance portion
of Original Medicare. Original Medicare allows
you to use any doctor, specialist, or hospital that
accepts Medicare. You do not need a referral. You
are paying a separate amount for each service, also
called fee for service.
Inpatient hospital care such as critical access
hospitals, inpatient rehabilitation facilities, and
long-term care hospitals.
Medicare Advantage Plans PA

Medicare Part B – Medical Insurance
Medicare Part B is the Medical Insurance portion
of Original Medicare. It is optional.
Original Medicare allows you to use any doctor,
specialist, or hospital that accepts Medicare.
You do not need a referral. You are paying a
separate amount for each service, also called fee for
service. Part B only pays for services determined to
be reasonable and necessary in the diagnosis and
treatment of a specific illness or injury.
Turning 65 Medicare

Dental Insurance Plans & Their Coverage
Dental discounts plan is NOT a dental insurance.
Rather, dental offices participating in the dental
discount plan agree to charge substantially
discounted prices for the majority of dental work.
This approach has several advantages versus
traditional dental insurance plans:
Medicare Under 65

Individual Supplemental Insurance Plans:
The Hospital Indemnity Plan is NOT a
comprehensive major medical plan. These plans
include benefits for hospital confinement and
specified medical and surgical events. Such plan
pays you the specified cash amount for each
covered service, regardless of what the provider
charges you. Cash benefits are paid directly to you
and may be used for any purpose. Normally, there
is no deductible with the Hospital Indemnity Plan.
Medigap Plan N PA

Planning for Long-Term Care (LTC)
LTC includes medical and non-medical care. Long-term
medical care is chronic care with an aim is to control and
maintain non life threatening ailments without an attempt
to permanently cure them (opposite of acute care when a
patient is treated in an attempt to be permanently cured).
The long-term non-medical care is custodial care designed
to assist with activities of daily living (ADL), such as
bathing, grooming, eating, dressing and other similar
activities, as well as with other less personal needs, such
as preparing meals, shopping, housework, etc.
