4. EM STP/CCG Area of Challenge
Improve care and reduce costs of care for patients with long-term
conditions, specifically diabetes, respiratory, cardiology and falls
With the following target impacts/outcome:
• To significantly reduce or eliminate of wasteful acute based outpatient
appointments for this cohort of patients;
• To increase efficiency of care for these LTCs in primary care
• To reduce instances of escalation and emergency admissions

5. Three solutions selected
 Falls prevention and management
 Demonstrator in Leicester, Leicestershire & Rutland
 Tender issued to select supplier for framework contract
 Demonstrator deployment building upon existing service improvement
 Intelligent outpatient scheduling and management
 Demonstrator in Derby Teaching Hospital
 Supplier selected
 GP decision and referral support system
 Further co-design work on-going
 Demonstrator planned in Lincolnshire
 Supplier selection to follow
All three solutions based upon digital systems

Sept-Oct ‘16: Confirm scope of initial challenge area
Oct-Nov ‘16: Procurement and governance design
Jan ‘17: Industry / healthcare engagement workshops
Feb-April ‘17: Co-production of Solutions
May ‘17: Solution selection (STPs / CCGs)
June-Aug‘17: Product requirements specification & host site

Sept-Oct‘17: Tendering, supplier selection and resource sign-off
Oct-Dec ‘17: Contracting and mobilisation of first demonstrators
2018: Demonstrator deployment, operation and evaluation
2019+: Spread and adoption under commercial contracts

7. Key lessons
Defining needs, priorities & target outcomes is key
Involving suppliers in co-design of solution at an early
stage is valuable
Demonstrator deployments (not pilots)
Draft business case at outset (evaluation to confirm)
Procurement, IG, etc addressed in timely manner
