3. ο TTV
Appears similar to HGV with respect to prevalence,
transmissibility, and lack of clinical significance
ο SEV-V
Distinctly associated to TTV, and so to transfusion
associated non-A through E
ο CMV and EBV
Generally mild, in absence to
Need special care in severe immunosupression
and some special category recipients
3
4. Post transfusion Hepatitis
HBV and HCV
ο Both establish prolonged carrier states in donors
characterized by high titer viremia in absence of
symptoms
ο Both cause long-term liver related mortality
4
11. Chronic carriers of HBV
ο HBsAg
ο Risk- Age dependent
ο β€ 5% infected in adults become chronic HBsAg
carriers
ο β₯90% infants infected parentally become chronic
carriers
ο HBsAg carriers
ο 450 million HBsAg carriers worldwide
ο Almost 10 % of these develop hepatic
insufficiency, cirrhosis, or HCC( Hepatocellular
carcinoma)
11
12. Chronic carriers of HCV
ο Most people infected once become chronic HCV
carriers
ο Almost 50% of them have biochemical and
histological evidence of chronic liver inflammation
ο Despite this most of these remain asymptomatic
ο Risk: 20% of patients may develop cirrhosis
within 20 years of chronic HCV infection, and up
to 1-5% may develop HCC if chronic HCV
persists for 20 years
12
13. Post transfusion HCV
ο Post transfusion HCV is mild in children and
resolves more frequently than adults
ο HCV prevalence in Saudi Arabia for children is
0.1% and for adults 0.4% to 1.7%
Shoobakshi et al: Saudi Med J 2003; 24(suppl
2): S81-6
ο HCV prevalence in Eastern province has shown
decline from 1.04% in 1998 to 0.59% in 2001
ο Bashawari et al: Clin Lab Haematol 2004; 26(3):
225-8
13
15. Hepatitis Markers
HBV
ο Incubation period 2-6 weeks
ο HBV-DNA
ο MP-NAT- first marker to appear (16-24 days)
ο ID-NAT can detect almost 2 weeks before MP-
NAT
ο HBsAg; 5-10 days later to HBV-DNA
ο Anti-HBc appears several weeks later first as IgM
and later IgG
ο The clearance of HBsAg and appears of anti-HBs
signals the resolution of infection if there is no
mutation in antigenic sites of HBV
15
16. Hepatitis Markers
ο Two other markers HBeAg and anti-HBe are
useful diagnostic and prognostic markers but not
employed in donor screening
ο Current EIA approximately detects about 0.2 to
0.7 ng/ml or β₯ 3x 107 of HBsAg particles
ο BioRad: HBsAg monilisa: detects 0.06 ng/ml
ο Architect (ChLIA detects-----)
ο NAT allows to detect 10 genomic copies of HBV-
DNA, but not all appears to be infectious.
16
17. HCV
ο Incubation period is up to 10 weeks
ο 85% of HCV infected individuals get cirrhosis
ο HCV positive estimated in blood donors is 0.5 to
1.5%, but only 0.3 are chronic HCV carriers and
the rest may be false positive

22. Blood Unit Screening
ο 1986-87 AABB required anti-HBc and ALT testing for
blood donors to prevent post transfusion non-A non-B
(NANB) hepatitis
ο Current Markers
ο HBV
ο HBsAg and anti-HBc total
ο Anti-HBs if anti-HBc total Positive
ο NAT
ο No test available for HBsAg mutant of HBV
ο HCV

23. Current risk of post transfusion
hepatitis B
ο Recent study showed HBV in Saudi Arabia with
an average of 0.22% for adults with wild variation
between various regions from 0.33% to 0.72%,
and for children 0.5%
Madani et a;: Trans R Soc Trop Hyg 2007; 101 (3):
278-83
23
24. Current risk of pos transfusion
hepatitis C
ο With NAT testing it has decreased dramatically
from 1 in 60,000 to 1 in in 100,000
ο NAT has reduced the residual risk of transfusion
associated HCV infection to β€ 1 in 2,000,000
components transfused
