  2. 2. Heart Failure Surgery and Treatment in IndiaHeart failure, also called congestive heart failure, is a condition in which theheart can no longer pump enough blood to the rest of the body. In thiscondition blood moves through the heart and body at a slower rate, andpressure in the heart increases. Therefore, the heart cannot pump enoughoxygen and nutrients to meet the bodys needs. The chambers of the heartrespond by stretching to hold more blood to pump through the body or bybecoming thicker and stiffer. This helps to keep the blood moving for a shortwhile, but then the heart muscle walls weaken and are not able to pump asstrongly. The heart muscle walls are damaged and do not pump or fillnormally. The kidneys often respond by causing the body to retain fluid (water)and sodium. If fluid builds up in the arms, legs, ankles, feet, lungs or otherorgans, the body becomes congested; congestive heart failure is the termused to describe this condition. Heart failure is the leading cause ofhospitalization in people over age 65.Symptoms of Heart FailureSymptoms of heart failure most often begin slowly. At first, they may only occur when you are very active. Over time, breathingproblems and other symptoms may be noticed even when you are resting. Common symptoms are: Shortness of breath with activity, or after lying down for a while Cough Swelling of feet and ankles Swelling of the abdomen Weight gain Irregular or rapid pulse Palpitations Difficulty sleeping Fatigue, weakness, faintness Loss of appetite, indigestionOther symptoms may include decreased alertness or concentration, decreased urine production, nausea and vomiting andincreased urge to urinate at night. Infants may sweat a lot. Some patients with heart failure have no symptoms. In these people, the symptoms may develop only with these conditions: Abnormal heart rhythm (arrhythmias) Anemia Hyperthyroidism Infections with high fever Kidney diseaseTypes of Heart Failuresymptoms Left-Sided Heart Failure - In left-sided or left ventricular (LV) heart failure, the left side of the heart must work harderto pump the same amount of blood. There are two types of left-sided heart failure. Drug treatments are different for the twotypes.
  3. 3. Systolic failure: The left ventricle loses its ability to contract normally. The heart cant pump with enough force to push enough blood into circulation. Diastolic failure or dysfunction: The left ventricle loses its ability to relax normally (because the muscle has become stiff). The heart cant properly fill with blood during the resting period between each beat.Right-Sided Heart Failure - Right-sided or right ventricular (RV) heart failureusually occurs as a result of left-sided failure. When the left ventricle fails, increased fluid pressure is, in effect, transferred back through the lungs, ultimately damaging the hearts right side. When the right side loses pumpingpower, blood backs up in the bodys veins..Congestive Heart Failure - Congestive heart failure is a type of heart failure which requires seeking timely medical attention, although sometimes the two terms are used interchangeably. A test called the ejection fraction (EF) is used to measure howwell your heart pumps with each beat to determine if systolic dysfunction or heart failure with preserved left ventricularfunction are present.Causes of Heart FailureHeart failure is caused by many things that damage the heart muscle, including: Coronary artery disease Heart Cardiomyopathy Heart defects Diabetes High blood pressure (hypertension) Arrhythmia (abnormal heart rhythms) Kidney disease Obesity (being overweight) Medications — some chemotherapy agentsHeart failure often occurs when several diseases or conditions are present at once.Test and Diagnosis of Heart FailureA physical examination may reveal the following: Fluid around the lungs (pleural effusion) Irregular heartbeat Leg swelling (edema) Neck veins that stick out (are distended) Swelling of the liverListening to the chest with a stethoscope may reveal lung crackles or abnormal heart sounds. This condition may also alterthe results of other tests which include blood chemistry, complete blood count, creatinine clearance, liver function tests, bloodtest (Uric acid, Sodium), Urinalysis.
  4. 4. The following tests may be used to diagnose or monitor heart failure: Chest x-ray ECG Echocardiogram Cardiac stress tests Heart CT scan Heart catheterization MRI of the heart Nuclear heart scansTreatment of Heart FailureHeart failure caused by damage to the heart that has developed over time cant be cured. But it can be treated, quite often withstrategies to improve symptoms.Medications Heart failure patients need multiple medications. Each one treats a different symptom or contributing factor. Its important to discuss all of the drugs you take with your doctor and understand their desired effects and possible side effects. Medicines decrease the clotting (coagulating) ability of the blood. Thus blood thinners or anti-coagulators are usually advised. They do not dissolve existing blood clots but are used to treat certain blood vessels, heart and lung conditions. Medicines may also prevent the clots from becoming larger and causing more serious problems. They are often prescribed to prevent first or recurrent stroke.Surgery of Heart Failure Surgical Procedures Without Devices Implantable Medical Devices Surgical Procedures Without Devices - Heart valve surgery, coronary bypass surgery (CABG), and angioplasty may help some people with heart failure. Implantable Medical Devices - The following devices may be recommended for certain patients with heart failure: A pacemaker to help treat slow heart rates or other heart signaling problems. A biventricular pacemaker to help the both sides of your heart contract at the same time; it is also called cardiac resynchronization therapy. An implantable cardioverter-defibrillator that recognizes life-threatening, abnormal heart rhythms and sends an electrical pulse to stop them.Other Treatment OptionsSevere heart failure may require the following treatments when other therapies no longer work. They are often used when aperson is waiting for a heart transplant: Intra-aortic balloon pump (IABP) Left ventricular assist device (LVAD)
  5. 5. Lifestyle ChangesThe patient will need to make some changes in his daily life to treat heart failure. Eat less salt. Be more active. Stay at a healthy weight, or lose weight if you need to. Even a few pounds can make a difference. Stop smoking. Limit alcohol. Control blood pressure. Control diabetes.Advantages of Heart Failure Surgery and Treatment in IndiaOnce the diagnosis of heart failure is established, a number of pharmacologic strategies are available to limit and reverse themanifestations of HF. In particular, mortality rates among patients with heart failure can be reduced drastically. Treatmentincreases survival and decreases repeat hospitalizations.Risks and Side Effects Associated with Heart Failure Surgery and TreatmentPossible side effects of medications include cough, gastrointestinal upset (such as nausea, heartburn, and diarrhea),headache, light-headedness and fainting, low blood pressure, muscle cramps. Possible Complications could be pulmonaryedema and total heart failure (circulatory collapse)Heart Failure Surgery and Treatment in IndiaThe doctors and researchers in India have actively contributed in developing new ways to diagnose and treat people who haveheart failure. Each year doctors internationally trained in heart care (cardiologists) evaluate and treat thousands of people whohave congestive heart failure. 8. 8. Knee Replacement Surgery in India Knee replacement surgery also known as knee arthroplasty is procedure to replace the weight-bearing surfaces of the knee joint to relieve the pain and disability of osteoarthritis. It may be performed for other knee diseases such as rheumatoid arthritis and psoriatic arthritis. The procedure has been proven to help individuals return back to moderately challenging activities such as golf, bicycling, and swimming. Total knees are not designed for jogging, or sports like tennis and skiing (although there certainly are people with total knee replacements that participate in such sports). Knee replacement is a routine surgery performed on over 600,000 people worldwide each year. Over 90% of people who have had Total Knee Replacement experience an improvement in knee pain and function.Knee Replacement Surgery CandidatesA person would be considered a candidate for knee replacement if there is: • Daily pain • The pain is severe enough to restrict work, recreation and ordinary activities of daily living • Significant stiffness in the knee • Significant instability (constant giving way) of the knee • Significant deformity (knock-knees or bow-legs) that hinders normal function of the knee • Damage from arthritic conditions, such as osteoarthritis, rheumatoid arthritis or post-traumatic arthritisSuccess Rate of Knee Replacement Surgery:Knee replacement surgery is recognized as a miracle of modern surgery. Most orthopedic experts consider replacement to bethe best method of handling arthritis in the knee. Knee replacements have literally put hundreds of thousands of Americansback on their feet and allowed them to enjoy their golden years.Knee Replacement Surgery Preparations: • If you smoke, cut down or quit. Smoking changes blood flow patterns, delays healing and slows recovery. • If you drink, dont have any alcohol for at least 48 hours before surgery. • Ask your doctor for pre-surgical exercises. If you are having hip or knee replacement surgery, doing exercises to strengthen your upper body will help you cope with crutches or a walker after surgery. • Your primary care physician or an internist will conduct a general medical evaluation several weeks before surgery. This examination will assess your health and your risk for anesthesia. The results of this examination should be forwarded to your orthopaedic surgeon, along with a surgical clearance. • Shortly before your scheduled surgery, you will probably have an orthopedic examination to review the procedure and answer any last-minute questions. • You may need to take several types of tests, including blood tests, a cardiogram, a urine sample and a chest X-ray. • Advice your surgeon of any medical conditions you have and of all the medications you are taking. You may need to stop taking certain medications or your surgeon may recommend substitute medications until your surgery. Medications such as corticosteroids, insulin or anti-coagulants will need to be managed before and after surgery.
