2. TAKBUHANG PISAY! Registration Form
Submit this form to your year level representative at the cafeteria area on March 3 to 4
(1pm onwards) in order to receive your singlet before the event. Late registrants on
March 10 to 11 will receive their singlets on the day itself.
Each Takbuhang Pisay!participant needs to pay P300 as registration fee for the runner's kit that
includes the singlet (running shirt) and bib.

PARTICIPANTS' WAIVER
I hereby state that : 1) |have secured a clearance from my physician that I am physically fit to
participate in the TAKBUHANG PISAY on March 19,2A11 or on any day that said race may be held; 2)
I have sufficiently prepared for this event, and that I am fit to join the event. Being aware of the risk
involved and in'consideration of the acceptance of my entry and the foregoing premises, I hereby
forever discharge, release, waive and quitclbim any and all causes of action, whether civil or criminal
against race organizers and sponsors, from any injury, sickness, death, accidenis and damages that I
