Activity: Ishikawa's Fishbone Diagram
The purpose of this exercise is to practice constructing and using fishbone diagrams. Below is a case situation. Using
the information in the case, develop fishbone diagram to accurately reflect the situation. Use this tool to analyze
the problem and make recommendations for solving it.
Situation
You work for CMHC, a small community mental health center which provides patient mental health services.
CMHC employs a variety of professionals including 10 social workers, 5 psychologists, 1 psychiatrist, an Executive
Director, Director of Clinical Services, and various support staff. Staff have increasingly complained about staff
meetings not being productive. Data collected from the suggestion box over the last 2 months show that many
people have commented about staff meetings. This data is presented below.
