In consideration of the plan to produce these tasks above
I hereby consent to the use of my
name, photographs, voice over and the company who is
directing and exhibiting these adverts, sponsorships and
pop up.
The production itself will be published on the websites
YouTube and Blogger. The sponsorship and pop up will
require at least two days of filming and photography. As
well as the two adverts requiring at least three days to be
filmed. The ancillary task and pop up will be located in
the studio. The adverts will be location based filming.
Therefore travel costs want be needed.
The adverts may be shot in holiday dates therefore dates
will need to be discussed and confirmed for availability
and timing.
I agree to be part of the pop up/ sponsorship/ adverts
and will be available for filming.
