PALLIATIVE CARE IS...
the total care of patients and
their families whose illness is not
responsive to curative treatment.
The primary focus is to control
pain and other symptoms, with
attention to psychological, social
and spiritual issues. The goal of
palliative care is achievement
of the best possible quality of
life for patients.
OUR VISION
Consistent with the mission and core
values of Saint Alphonsus Regional
Medical Center, the Palliative Care
Program will provide persons with
life-limiting illness, their families and
caregivers with the support, guidance
and tools needed to maintain optimum
comfort and well-being along life’s
journey when cure of an illness may
not be a possibility.

Center in Trinity Health in
the spirit of the Gospel, to
heal body, mind and spirit,
to improve the health of our
communities and to steward
the resources entrusted to us.
2. OUR PROGRAM
Consultation is provided by an
interdisciplinary team.
Unlike hospice, which provides care to the
terminally ill usually during the last six
months of life, palliative care consultation
may be provided over an extended time
period to any patient whose illness is no
longer receiving curative treatments.
OUR CONSULTATION
TEAM INCLUDES...
x a physician, who specializes in
palliative care management of
pain and other symptoms through
medications or treatments
x a nurse, who ensures that the patient
is properly cared for by coordinating
with other caregivers both in the
hospital and at home, including
teaching family members how to
care for the patient
x a social worker, who discusses social
issues and lifestyle changes with the
patient and family members, and who
helps with planning the discharge of
the patient and arranging the aid of
community resources
x a chaplain, who is available to
patients and families for spiritual and
emotional needs, respecting their
faith traditions.

and complex problems

symptom management

enabled, and empowered
x Keeps patients informed of their rights
x Allows patients to direct their care,
centering on the goals of the
patients and their families
x Provides continuity of care and
accessibility within systems and
among providers
x Provides specifically trained staff
persons in the various aspects of
palliative care
x Communicates with patient’s doctor
to manage symptoms, if needed
x Assists patients and families with end
of life concerns, decision making,
resources, referrals, and bereavement
counseling as needed
