proof and that more care is required in order to prevent re-occurrence .

QHSE training and reporting activities target set to 95% has been exceeded for the past
year. Nonetheless, all efforts should be engaged in order to reach maximal effectiveness

of QHSE activities.
9. Lessons learned from past experience
During hammering, one of our site employees hit his left hand pointer finger with the hammer
he was using in his right hand. The result was a cut on the finger caused by hit.
The incident has been reported, the IP has received medical care and returned to worksite.
The employee has received a QHSE refresher course the following day, targeting usage of hand
tools and proper work positions. Following this incident, all the other worksite employees have
been given a refreshment course on the same subject and no other similar situations occurred.
