ASSESSMENT AND MANAGAEMENT OF
AUDITORY PROBLEMS
ANATOMY OF THE EXTERNAL EAR
• THE EXTERNAL EAR INCLUDES THE AURICLE (PINNA) AND
THE EXTERNAL AUDITORY CANAL THE EXTERNAL EAR IS
SEPARATED FROM THE MIDDLE EAR BY A DISKLIKE
STRUCTURE CALLED THE TYMPANIC MEMBRANE (EARDRUM).
• AURICLE
• THE AURICLE, ATTACHED TO THE SIDE OF THE HEAD BY SKIN,
IS COMPOSED MAINLY OF CARTILAGE, EXCEPT FOR THE FAT
AND SUBCUTANEOUS TISSUE IN THE EARLOBE. THE AURICLE
COLLECTS THE SOUND WAVES AND DIRECTS VIBRATIONS
INTO THE EXTERNAL AUDITORY CANAL
• EXTERNAL AUDITORY CANAL
• THE EXTERNAL AUDITORY CANAL IS APPROXIMATELY 2.5 CM
LONG. THE LATERAL THIRD IS AN ELASTIC CARTILAGINOUS
AND DENSE FIBROUS FRAMEWORK TO WHICH THIN SKIN IS
ATTACHED. THE MEDIAL TWO THIRDS IS BONE LINED WITH
THIN SKIN. THE EXTERNAL AUDITORY CANAL ENDS AT THE
TYMPANIC MEMBRANE. THE SKIN OF THE CANAL CONTAINS
HAIR, SEBACEOUS GLANDS, AND CERUMINOUS GLANDS,
WHICH SECRETE A BROWN, WAXLIKE SUBSTANCE CALLED
CERUMEN (EAR WAX). THE EAR’S SELF-CLEANING
MECHANISM MOVES OLD SKIN CELLS AND CERUMEN TO THE
OUTER PART OF THE EAR.
• ANATOMY OF THE MIDDLE EAR
• THE MIDDLE EAR, AN AIR-FILLED CAVITY, INCLUDES THE TYMPANIC
MEMBRANE LATERALLY AND THE OTIC CAPSULE MEDIALLY. THE MIDDLE
EAR CLEFT LIES BETWEEN THE TWO. THE MIDDLE EAR IS CONNECTED TO
THE NASOPHARYNX BY THE EUSTACHIAN TUBE AND IS CONTINUOUS WITH
AIR FILLED CELLS IN THE ADJACENT MASTOID PORTION OF THE
TEMPORAL BONE
• THE EUSTACHIAN TUBE, WHICH IS APPROXIMATELY 1 MM WIDE AND 35 MM
LONG, CONNECTS THE MIDDLE EAR TO THE NASOPHARYNX. NORMALLY,
THE EUSTACHIAN TUBE IS CLOSED, BUT IT OPENS BY ACTION OF THE
TENSOR VELI PALATINI MUSCLE WHEN THE PERSON PERFORMS A
VALSALVA MANEUVER, YAWNS, OR SWALLOWS. IT DRAINS NORMAL AND
ABNORMAL SECRETIONS OF THE MIDDLE EAR AND EQUALIZES
PRESSURE IN THE MIDDLE EAR WITH THAT OF THE ATMOSPHERE.
• TYMPANICMEMBRANE
• THE TYMPANIC MEMBRANE (EARDRUM), ABOUT 1 CM IN DIAMETER AND VERY THIN, IS
NORMALLY PEARLY GRAY AND TRANSLUCENT. IT CONSISTS OF THREE LAYERS OF
TISSUE: AN OUTER LAYER, CONTINUOUS WITH THE SKIN OF THE EAR CANAL; A FIBROUS
MIDDLE LAYER; AND AN INNER MUCOSAL LAYER, CONTINUOUS WITH THE LINING OF THE
MIDDLE EAR CAVITY. APPROXIMATELY 80% OF THE TYMPANIC MEMBRANE IS
COMPOSED OF ALL THREE LAYERS AND IS CALLED THE PARS TENSA. THE REMAINING
20% LACKS THE MIDDLE LAYER AND IS CALLED THE PARS FLACCIDA. THE ABSENCE OF
THIS FIBROUS MIDDLE LAYER MAKES THE PARS FLACCIDA MORE VULNERABLE TO
PATHOLOGIC DISORDERS THAN THEPARS TENSA
• THE TYMPANIC MEMBRANE PROTECTS THE MIDDLE EAR AND CONDUCTS SOUND
VIBRATIONS FROM THE EXTERNAL CANAL TO THE OSSICLES. THE SOUND PRESSURE IS
MAGNIFIED 22 TIMES AS A RESULT OF TRANSMISSION FROM A LARGER AREA TO A
SMALLERONE.
• OSSICLES:THE MIDDLE EAR CONTAINS THE THREE SMALLEST BONES (THE
OSSICLES)OF THE BODY: THE MALLEUS, THE INCUS, AND THE STAPES. THE OSSICLES,
WHICH ARE HELD IN PLACE BY JOINTS, MUSCLES, AND LIGAMENTS, ASSIST IN THE
TRANSMISSION OF SOUND. TWO SMALL FENESTRAE (OVAL AND ROUND WINDOWS),
LOCATED IN THE MEDIAL WALL OF THE MIDDLE EAR, SEPARATE THE MIDDLE EAR FROM
THE INNER EAR. THE FOOTPLATE OF THE STAPES SITS IN THE OVAL WINDOW, SECURED
BY A FIBROUS ANNULUS (RING-SHAPED STRUCTURE). THE FOOTPLATE TRANSMITS
SOUND TO THE INNER EAR. THE ROUND WINDOW, COVERED BY A THIN MEMBRANE,
PROVIDES AN EXITFOR SOUND VIBRATIONS
ANATOMY OF THE INNER EAR
• THE INNER EAR IS HOUSED DEEP WITHIN THE TEMPORAL BONE. THE
ORGANS FOR HEARING (COCHLEA) AND BALANCE (SEMICIRCULAR
CANALS), AS WELL AS CRANIAL NERVES VII (FACIAL NERVE) AND VIII
(VESTIBULOCOCHLEAR NERVE), ARE ALL PART OF THIS COMPLEX
ANATOMY. THE COCHLEA AND SEMICIRCULAR CANALS ARE HOUSED IN
THE BONY LABYRINTH. THE BONY LABYRINTH SURROUNDS AND
PROTECTS THE MEMBRANOUS LABYRINTH, WHICH IS BATHED IN A FLUID
CALLED PERILYMPH.
• MEMBRANOUSLABYRINTH
• THE MEMBRANOUS LABYRINTH IS COMPOSED OF THE UTRICLE, THE
SACCULE, THE COCHLEAR DUCT, THE SEMICIRCULAR CANALS, AND THE
ORGAN OF CORTI, ALL OF WHICH ARE SURROUNDED BY A FLUID
CALLEDENDOLYMPH. THE THREE SEMICIRCULAR CANALS—POSTERIOR,
SUPERIOR, AND LATERAL, WHICH LIE AT 90-DEGREE ANGLES TO ONE
ANOTHER—CONTAIN SENSORY RECEPTOR ORGANS, ARRANGED TO
DETECT ROTATIONAL MOVEMENT. THESE RECEPTOR END ORGANS ARE
STIMULATED BY CHANGES IN THE RATE OR DIRECTION OF A PERSON’S
MOVEMENT. THE UTRICLE AND SACCULE ARE INVOLVED WITH LINEAR
MOVEMENTS.
• ORGANOFCORTI
• THE ORGAN OF CORTI IS HOUSED IN THE COCHLEA, A SNAIL-SHAPED,
BONY TUBE ABOUT 3.5 CM LONG WITH TWO AND A HALF SPIRA TURNS.
THE ORGAN OF CORTI, ALSO CALLED THE END ORGAN FOR HEARING,
TRANSFORMS MECHANICAL ENERGY INTO NEURAL ACTIVITY AND
SEPARATES SOUNDS INTO DIFFERENT FREQUENCIES. THIS
ELECTROCHEMICAL IMPULSE TRAVELS THROUGH THE ACOUSTIC NERVE
TO THE TEMPORAL CORTEX OF THE BRAIN TO BE INTERPRETED AS
MEANINGFUL SOUND.
FUNCTION OF THE EARS
• HEARING
• HEARING IS CONDUCTED OVER TWO PATHWAYS: AIR AND BONE. SOUNDS
TRANSMITTED BY AIR CONDUCTION TRAVEL OVER THE AIRFILLED
EXTERNAL AND MIDDLE EAR THROUGH VIBRATION OF THE TYMPANIC
MEMBRANE AND OSSICLES. SOUNDS TRANSMITTED BY BONE
CONDUCTION TRAVEL DIRECTLY THROUGH BONE TO THE INNER EAR,
BYPASSING THE TYMPANIC MEMBRANE AND OSSICLES. NORMALLY, AIR
CONDUCTIONIS THE MORE EFFICIENT PATHWAY
• THE ROLE OF THE EXTERNAL AND MIDDLE PART OF THE EAR IS TO
CONDUCT AND AMPLIFY SOUND WAVES FROM THE ENVIRONMENT. THIS
PART OF SOUND CONDUCTION IS TERMED AIR CONDUCTION. PROBLEMS
IN THESE TWO PARTS OF THE EAR MAY CAUSE CONDUCTIVE HEARING
LOSS, RESULTING IN A DECREASE IN SOUND INTENSITY AND/OR A
DISTORTION IN SOUND
• DISTURBANCES IN EQUILIBRIUM CAN IMPAIR COORDINATION, BALANCE,
AND ORIENTATION. DAMAGE TO OR AN ABNORMALITY OF THE INNER EAR
OR ALONG THE NERVE PATHWAYS RESULTS IN SENSORINEURAL
HEARING LOSS. IN ADDITION TO CAUSING DISTORTION OR FAINTNESS OF
SOUND, SENSORINEURAL HEARING LOSS MAY AFFECT THE ABILITY TO
UNDERSTAND SPEECH OR CAUSE COMPLETE HEARING LOSS.
IMPAIRMENT WITHIN THE AUDITORY PATHWAYS OF THE BRAIN
CAUSESCENTRAL HEARING LOSS. THIS TYPE OF HEARING LOSS CAUSES
DIFFICULTY IN UNDERSTANDING THE MEANING OF WORDS THE PERSON
HEARD
• BALANCE ANDEQUILIBRIUM
• BODY BALANCE IS MAINTAINED BY THE COOPERATION OF THE MUSCLES
AND JOINTS OF THE BODY (PROPRIOCEPTIVE SYSTEM), THE EYES (VISUAL
SYSTEM), AND THE LABYRINTH (VESTIBULAR SYSTEM). THESE AREAS
SEND THEIR INFORMATION ABOUT EQUILIBRIUM, OR BALANCE, TO THE
BRAIN (CEREBELLAR SYSTEM). THE VESTIBULAR APPARATUS OF THE
INNER EAR PROVIDES FEEDBACK REGARDING THE MOVEMENTS AND
THE POSITION OF THE HEAD AND BODYIN SPACE.
EFFECTS OF AGING ON AUDITORY SYSTEM
• PRESBYCUSIS, OR HEARING LOSS DUE TO AGING, IS THE THIRD MOST COMMON HEALTH
ISSUE IN OLDER ADULTS.SOUND TRANSMISSION IS DIMINISHED BY CALCIFICATION OF
THE OSSICLES. TINNITUS, OR RINGING IN THE EARS, MAY ACCOMPANY THE HEARING
LOSS THAT RESULTS FROM THE AGING PROCESS. CERUMEN GLANDS ATROPHY,
CAUSING CERUMEN(EARWAX) TO BE MUCH DRIER. THE HAIR IN THE EAR BECOMES
THICKER ANDCOARSER, ENTRAPPING THE HARD, DRY CERUMEN IN THE CANAL.
ACCUMULATIONOF DRY CERUMEN IN THE EXTERNAL CANAL CAN INTERFERE WITH
THETRANSMISSION OF SOUND. A DECLINE IN BALANCE RESULTS FROM ATROPHY
OFVESTIBULAR STRUCTURES IN THE INNER EAR, SLOWING OF MOTOR RESPONSES,
ANDMUSCULOSKELETALLIMITATIONS
ASSESSMENT OF AUDITORY SYSTEM
• Assessment of the auditory system should include assessment of hearing
and equilibrium
• Subjective Data
Past Health History
• Ask the patient about previous ear problems, especially during Childhood:
(1) frequency of acute middle ear infections(otitis media);
(2) surgical procedures (e.g., myringotomy);
(3)perforations of the eardrum;
(4) drainage;
(5) history of mumps,measles, or scarlet fever.
. Assess for systemic conditions, including hypertension, rheumatoid
arthritis, and inflammatory bowel disease,that are associated with hearing
loss.
• head injury because it may result in hearing loss
• RECORD SYMPTOMS SUCH AS VERTIGO, TINNITUS, AND HEARING LOSS IN THE
PATIENT’S WORDS. VERTIGO IS A SENSE THAT THE PERSON OR OBJECTS AROUND THE
PERSON ARE MOVING OR SPINNING. IT IS USUALLY STIMULATED BY HEAD MOVEMENT.
DIZZINESS IS A SENSATION OF BEING OFF-BALANCE THAT OCCURS WHEN STANDING
OR WALKING. NYSTAGMUS IS A ABNORMAL EYE MOVEMENT THAT MAY BE SEEN AS A
TWITCHING OF THE EYEBALL OR DESCRIBED BY THE PATIENT AS A BLURRING OF VISION
WITH HEAD OR EYE MOVEMENT
• Medications
• Obtain information about present or past use of
ototoxic drugs (cause damage to CN VIII). These
include aspirin, some antibiotics (aminoglycosides,
erythromycin, vancomycin), loop diuretics,
nonsteroidal antiinflammatory drugs (NSAIDs),
antimalarial agents, and certain chemotherapy drugs
(e.g., bleomycin, cyclophosphamide,cisplatin).
• Surgery or Other Treatments
• Document previous hospitalizations for ear surgery, including
myringotomy (ventilation holes placed in the TM with or
without tubes), tympanoplasty (surgical repair of TM),
tonsillectomy, and adenoidectomy. Record the use of and
satisfaction with a hearing aid.Note any problems with
impacted cerumen.
• Objective Data

Indicators of Possible Hearing Loss
• Does not respond to or understand oral
communication
• Has excessively loud or soft speech
• Answers questions inappropriately
• Tilts head, leans forward when listening
• Constantly needs to clarify conversation
• States other people mumble all the time
• Increases the volume of radio or TV
• Difficulty hearing over the phone
• EXTERNA EARS:
• SIZE AND SHAPE OF PINNA AND ANGLE OF ATTACHEMENT TO HEAD
• SIGNS OF TRAUMA
• SUSPICIOUS SKIN LESIONS
• INFECTION/ DISCHARGE
• SCARS OF PREVIOUS SURGERY
• PALPATE THE AURICLE AND MASTOID AREAS FOR TENDERNESS AND NODULES
• EXTERNAL AUDITORY CANAL AND TYMPANUM
• Inspect the TM for color, fluid behind the membrane,
landmarks, contour, and intactness The TM is normally pearl
gray, white, or pink; shiny; and translucent. The handle
(manubrium) of the malleus and its short process (umbo)
should be visible through the membrane. The position and
dome (concave) shape of the TM cause the light from the
otoscope to reflect in a cone shape with crisp edges. If the
TM is bulging or retracted, the edges of the light reflex will be
fuzzy (diffuse) and may spread over the TM. The middle and
inner ear cannot be examined with the otoscope

DIAGNOSTIC STUDIES OF AUDITORY SYSTEM
• PURE TONE AUDIOMETRY
• The audiometer produces pure tones at varying intensities to
which the patient can respond. Hearing loss can affect
certain sound frequencies. The specific pattern produced on
the audiogram by these losses can help in diagnosing the
type of hearing loss. Hertz (Hz) is the unit of measurement
used to classify the frequency of a tone. The intensity or
strength of asound wave is expressed in terms of decibels
(dB), ranging from 0 to 110 dB.
• Normal speech is around 40 to 65 dB; a soft whisper is 20
dB. Normally, a child and a young adult can hear frequencies
from about 16 to 20,000 Hz, but hearing is most sensitive
between 500 and 4000 Hz. This is similar to normal speech
frequencies. A 40- to 45-dB loss in these frequencies causes
moderate difficulty in hearing normal speech

BETWEEN THE AIR PRESSURE IN TH AIR CANAL AND THE MOVEMENT OF
TH EARDRUM AND OSSICLES.A SMALL PROBE IS PLACED IN THE EAR
AND AIR PRESSURE IN THE EAR CANAL IS VARIED.EAR CANAL VOLUME
INDICATES WHWTER THE TM IS INACT OR IF ANY FLUID IS PRESENT.
• AUDITORY BRAIN STEM RESPONSE (ABR) TESTING:DETECTS SENSORY
NEURAL HEARING LOSS.ELECTRODES ARE PALCED ON THE SCALP AND
EARLOBES. BRAIN RESPONSES TO THE CLICKING SOUND SENT THROUGH
EAR PHONES ARE RECORDED.
• ELECTRONYSTAGMOGRAPHY: ENG IS THE MEASUREMENT OF GRAPHIC
RECORDING OF THE CHANGES IN ELECTRICAL POTENTIALS CREATED BY
EYE MOVEMENTS DURING SPONTANEOUS , POSITIONAL,OR EVKED
NYSTAGMUS. IT IS USED TO CLINICALLY EVALUATE PATIENTS WITH
DIZZINESS, VERTIGO OR BALANCE DYSFUNCTION
• MIDDLE EAR ENDOSCOPY: IS A PROCEDURE TO EVALUATE SUSPECTED
PERILYMPHATIC FISTULA AND NEW ONSET CONDUCTIVE HEARING
LOSS.TM IS ANESTHETISED TOPICALLY , EAR CANAL IS IRRIGATED WITH
STERILE NS. UNDER MICROSCOPIC GUIDANCETYMPANOTOMY IS CREATED
WITH LASER BEAM OR MYRINGOTOMY KNIEF AND ENDOSCOPE IS
INSERTED.

THE AURICLE COLLECTS THE SOUND WAVES AND DIRECTS VIBRATIONS INTO THE EXTERNAL AUDITORY CANAL
  • 3. • EXTERNAL AUDITORY CANAL • THE EXTERNAL AUDITORY CANAL IS APPROXIMATELY 2.5 CM LONG. THE LATERAL THIRD IS AN ELASTIC CARTILAGINOUS AND DENSE FIBROUS FRAMEWORK TO WHICH THIN SKIN IS ATTACHED. THE MEDIAL TWO THIRDS IS BONE LINED WITH THIN SKIN. THE EXTERNAL AUDITORY CANAL ENDS AT THE TYMPANIC MEMBRANE. THE SKIN OF THE CANAL CONTAINS HAIR, SEBACEOUS GLANDS, AND CERUMINOUS GLANDS, WHICH SECRETE A BROWN, WAXLIKE SUBSTANCE CALLED CERUMEN (EAR WAX). THE EAR’S SELF-CLEANING MECHANISM MOVES OLD SKIN CELLS AND CERUMEN TO THE OUTER PART OF THE EAR.

  • 5. • ANATOMY OF THE MIDDLE EAR • THE MIDDLE EAR, AN AIR-FILLED CAVITY, INCLUDES THE TYMPANIC MEMBRANE LATERALLY AND THE OTIC CAPSULE MEDIALLY. THE MIDDLE EAR CLEFT LIES BETWEEN THE TWO. THE MIDDLE EAR IS CONNECTED TO THE NASOPHARYNX BY THE EUSTACHIAN TUBE AND IS CONTINUOUS WITH AIR FILLED CELLS IN THE ADJACENT MASTOID PORTION OF THE TEMPORAL BONE • THE EUSTACHIAN TUBE, WHICH IS APPROXIMATELY 1 MM WIDE AND 35 MM LONG, CONNECTS THE MIDDLE EAR TO THE NASOPHARYNX. NORMALLY, THE EUSTACHIAN TUBE IS CLOSED, BUT IT OPENS BY ACTION OF THE TENSOR VELI PALATINI MUSCLE WHEN THE PERSON PERFORMS A VALSALVA MANEUVER, YAWNS, OR SWALLOWS. IT DRAINS NORMAL AND ABNORMAL SECRETIONS OF THE MIDDLE EAR AND EQUALIZES PRESSURE IN THE MIDDLE EAR WITH THAT OF THE ATMOSPHERE.
  • 6. • TYMPANICMEMBRANE • THE TYMPANIC MEMBRANE (EARDRUM), ABOUT 1 CM IN DIAMETER AND VERY THIN, IS NORMALLY PEARLY GRAY AND TRANSLUCENT. IT CONSISTS OF THREE LAYERS OF TISSUE: AN OUTER LAYER, CONTINUOUS WITH THE SKIN OF THE EAR CANAL; A FIBROUS MIDDLE LAYER; AND AN INNER MUCOSAL LAYER, CONTINUOUS WITH THE LINING OF THE MIDDLE EAR CAVITY. APPROXIMATELY 80% OF THE TYMPANIC MEMBRANE IS COMPOSED OF ALL THREE LAYERS AND IS CALLED THE PARS TENSA. THE REMAINING 20% LACKS THE MIDDLE LAYER AND IS CALLED THE PARS FLACCIDA. THE ABSENCE OF THIS FIBROUS MIDDLE LAYER MAKES THE PARS FLACCIDA MORE VULNERABLE TO PATHOLOGIC DISORDERS THAN THEPARS TENSA
  • 7. • THE TYMPANIC MEMBRANE PROTECTS THE MIDDLE EAR AND CONDUCTS SOUND VIBRATIONS FROM THE EXTERNAL CANAL TO THE OSSICLES. THE SOUND PRESSURE IS MAGNIFIED 22 TIMES AS A RESULT OF TRANSMISSION FROM A LARGER AREA TO A SMALLERONE. • OSSICLES:THE MIDDLE EAR CONTAINS THE THREE SMALLEST BONES (THE OSSICLES)OF THE BODY: THE MALLEUS, THE INCUS, AND THE STAPES. THE OSSICLES, WHICH ARE HELD IN PLACE BY JOINTS, MUSCLES, AND LIGAMENTS, ASSIST IN THE TRANSMISSION OF SOUND. TWO SMALL FENESTRAE (OVAL AND ROUND WINDOWS), LOCATED IN THE MEDIAL WALL OF THE MIDDLE EAR, SEPARATE THE MIDDLE EAR FROM THE INNER EAR. THE FOOTPLATE OF THE STAPES SITS IN THE OVAL WINDOW, SECURED BY A FIBROUS ANNULUS (RING-SHAPED STRUCTURE). THE FOOTPLATE TRANSMITS SOUND TO THE INNER EAR. THE ROUND WINDOW, COVERED BY A THIN MEMBRANE, PROVIDES AN EXITFOR SOUND VIBRATIONS
  • 8. ANATOMY OF THE INNER EAR • THE INNER EAR IS HOUSED DEEP WITHIN THE TEMPORAL BONE. THE ORGANS FOR HEARING (COCHLEA) AND BALANCE (SEMICIRCULAR CANALS), AS WELL AS CRANIAL NERVES VII (FACIAL NERVE) AND VIII (VESTIBULOCOCHLEAR NERVE), ARE ALL PART OF THIS COMPLEX ANATOMY. THE COCHLEA AND SEMICIRCULAR CANALS ARE HOUSED IN THE BONY LABYRINTH. THE BONY LABYRINTH SURROUNDS AND PROTECTS THE MEMBRANOUS LABYRINTH, WHICH IS BATHED IN A FLUID CALLED PERILYMPH.
  • 9. • MEMBRANOUSLABYRINTH • THE MEMBRANOUS LABYRINTH IS COMPOSED OF THE UTRICLE, THE SACCULE, THE COCHLEAR DUCT, THE SEMICIRCULAR CANALS, AND THE ORGAN OF CORTI, ALL OF WHICH ARE SURROUNDED BY A FLUID CALLEDENDOLYMPH. THE THREE SEMICIRCULAR CANALS—POSTERIOR, SUPERIOR, AND LATERAL, WHICH LIE AT 90-DEGREE ANGLES TO ONE ANOTHER—CONTAIN SENSORY RECEPTOR ORGANS, ARRANGED TO DETECT ROTATIONAL MOVEMENT. THESE RECEPTOR END ORGANS ARE STIMULATED BY CHANGES IN THE RATE OR DIRECTION OF A PERSON’S MOVEMENT. THE UTRICLE AND SACCULE ARE INVOLVED WITH LINEAR MOVEMENTS.

THE ORGAN OF CORTI, ALSO CALLED THE END ORGAN FOR HEARING, TRANSFORMS MECHANICAL ENERGY INTO NEURAL ACTIVITY AND SEPARATES SOUNDS INTO DIFFERENT FREQUENCIES. THIS ELECTROCHEMICAL IMPULSE TRAVELS THROUGH THE ACOUSTIC NERVE TO THE TEMPORAL CORTEX OF THE BRAIN TO BE INTERPRETED AS MEANINGFUL SOUND.

  • 12. • THE ROLE OF THE EXTERNAL AND MIDDLE PART OF THE EAR IS TO CONDUCT AND AMPLIFY SOUND WAVES FROM THE ENVIRONMENT. THIS PART OF SOUND CONDUCTION IS TERMED AIR CONDUCTION. PROBLEMS IN THESE TWO PARTS OF THE EAR MAY CAUSE CONDUCTIVE HEARING LOSS, RESULTING IN A DECREASE IN SOUND INTENSITY AND/OR A DISTORTION IN SOUND
  • 13. • DISTURBANCES IN EQUILIBRIUM CAN IMPAIR COORDINATION, BALANCE, AND ORIENTATION. DAMAGE TO OR AN ABNORMALITY OF THE INNER EAR OR ALONG THE NERVE PATHWAYS RESULTS IN SENSORINEURAL HEARING LOSS. IN ADDITION TO CAUSING DISTORTION OR FAINTNESS OF SOUND, SENSORINEURAL HEARING LOSS MAY AFFECT THE ABILITY TO UNDERSTAND SPEECH OR CAUSE COMPLETE HEARING LOSS. IMPAIRMENT WITHIN THE AUDITORY PATHWAYS OF THE BRAIN CAUSESCENTRAL HEARING LOSS. THIS TYPE OF HEARING LOSS CAUSES DIFFICULTY IN UNDERSTANDING THE MEANING OF WORDS THE PERSON HEARD
  • 14. • BALANCE ANDEQUILIBRIUM • BODY BALANCE IS MAINTAINED BY THE COOPERATION OF THE MUSCLES AND JOINTS OF THE BODY (PROPRIOCEPTIVE SYSTEM), THE EYES (VISUAL SYSTEM), AND THE LABYRINTH (VESTIBULAR SYSTEM). THESE AREAS SEND THEIR INFORMATION ABOUT EQUILIBRIUM, OR BALANCE, TO THE BRAIN (CEREBELLAR SYSTEM). THE VESTIBULAR APPARATUS OF THE INNER EAR PROVIDES FEEDBACK REGARDING THE MOVEMENTS AND THE POSITION OF THE HEAD AND BODYIN SPACE.

EFFECTS OF AGING ON AUDITORY SYSTEM • PRESBYCUSIS, OR HEARING LOSS DUE TO AGING, IS THE THIRD MOST COMMON HEALTH ISSUE IN OLDER ADULTS.SOUND TRANSMISSION IS DIMINISHED BY CALCIFICATION OF THE OSSICLES. TINNITUS, OR RINGING IN THE EARS, MAY ACCOMPANY THE HEARING LOSS THAT RESULTS FROM THE AGING PROCESS. CERUMEN GLANDS ATROPHY, CAUSING CERUMEN(EARWAX) TO BE MUCH DRIER. THE HAIR IN THE EAR BECOMES THICKER ANDCOARSER, ENTRAPPING THE HARD, DRY CERUMEN IN THE CANAL. ACCUMULATIONOF DRY CERUMEN IN THE EXTERNAL CANAL CAN INTERFERE WITH THETRANSMISSION OF SOUND. A DECLINE IN BALANCE RESULTS FROM ATROPHY OFVESTIBULAR STRUCTURES IN THE INNER EAR, SLOWING OF MOTOR RESPONSES, ANDMUSCULOSKELETALLIMITATIONS

ASSESSMENT OF AUDITORY SYSTEM • Assessment of the auditory system should include assessment of hearing and equilibrium • Subjective Data Past Health History • Ask the patient about previous ear problems, especially during Childhood: (1) frequency of acute middle ear infections(otitis media); (2) surgical procedures (e.g., myringotomy); (3)perforations of the eardrum; (4) drainage; (5) history of mumps,measles, or scarlet fever. . Assess for systemic conditions, including hypertension, rheumatoid arthritis, and inflammatory bowel disease,that are associated with hearing loss. • head injury because it may result in hearing loss
  • 17. • RECORD SYMPTOMS SUCH AS VERTIGO, TINNITUS, AND HEARING LOSS IN THE PATIENT’S WORDS. VERTIGO IS A SENSE THAT THE PERSON OR OBJECTS AROUND THE PERSON ARE MOVING OR SPINNING. IT IS USUALLY STIMULATED BY HEAD MOVEMENT. DIZZINESS IS A SENSATION OF BEING OFF-BALANCE THAT OCCURS WHEN STANDING OR WALKING. NYSTAGMUS IS A ABNORMAL EYE MOVEMENT THAT MAY BE SEEN AS A TWITCHING OF THE EYEBALL OR DESCRIBED BY THE PATIENT AS A BLURRING OF VISION WITH HEAD OR EYE MOVEMENT
  • 18. • Medications • Obtain information about present or past use of ototoxic drugs (cause damage to CN VIII). These include aspirin, some antibiotics (aminoglycosides, erythromycin, vancomycin), loop diuretics, nonsteroidal antiinflammatory drugs (NSAIDs), antimalarial agents, and certain chemotherapy drugs (e.g., bleomycin, cyclophosphamide,cisplatin).
  • 19. • Surgery or Other Treatments • Document previous hospitalizations for ear surgery, including myringotomy (ventilation holes placed in the TM with or without tubes), tympanoplasty (surgical repair of TM), tonsillectomy, and adenoidectomy. Record the use of and satisfaction with a hearing aid.Note any problems with impacted cerumen.

  • 21. • EXTERNA EARS: • SIZE AND SHAPE OF PINNA AND ANGLE OF ATTACHEMENT TO HEAD • SIGNS OF TRAUMA • SUSPICIOUS SKIN LESIONS • INFECTION/ DISCHARGE • SCARS OF PREVIOUS SURGERY • PALPATE THE AURICLE AND MASTOID AREAS FOR TENDERNESS AND NODULES • EXTERNAL AUDITORY CANAL AND TYMPANUM
  • 22. • Inspect the TM for color, fluid behind the membrane, landmarks, contour, and intactness The TM is normally pearl gray, white, or pink; shiny; and translucent. The handle (manubrium) of the malleus and its short process (umbo) should be visible through the membrane. The position and dome (concave) shape of the TM cause the light from the otoscope to reflect in a cone shape with crisp edges. If the TM is bulging or retracted, the edges of the light reflex will be fuzzy (diffuse) and may spread over the TM. The middle and inner ear cannot be examined with the otoscope
