1. It is recommended that sporting, recreation and leisure associations have their members/participants complete some form of
application prior to the commencement of the activity. This document should capture relevant information the entity needs for
recording the member/participant details. In addition, there should be some acknowledgment by the member/participant of
particular points that relate to the sport/activity of the entity.
Membership applications can be made online in some electronic format or in hard copy. In both scenarios it is important
that the entity stores a copy of the document and is able to retrieve it in case it is required for legal reasons.
The following points should be considered when putting together a membership or participant document.
1. WARNING AND ACKNOWLEDGMENT
Acknowledgment that the sport/activity has inherent risks and that injuries / losses could occur.
2. RELEASE AND INDEMNITY
Agree to waive rights against the organisation including, directors, volunteers etc.
(including transport by ambulance).
4. INSURANCE COVER
Acknowledge that the sport/activity has some limited group personal accident and liability insurance policies in place and that
the details of cover can be viewed by website /contacting someone /given a copy. Agreement that the individual should
consider their own financial circumstances and consider taking out private health insurance, life insurance, income protection
insurance, property insurance etc. Acknowledgement that if a claim is made the participant is responsible for paying
any policy excess.
5. FITNESS TO BE INVOLVED
Acknowledge that the member/participant is medically and physically fit to be involved in the activity and will notify the
organisation immediately if their fitness/ medical condition changes.
6. ABIDE BY RULES
Acknowledge that they will abide to the organisations code of behaviour and rules which are available on website/contacting
someone/given a copy.
7. RIGHTS TO USE IMAGES
Acknowledge and consent that photographs and electronic images may be taken for promotional purposes of the organisation.
8. PRIVACY
Acknowledge that personal information may be given to a relevant third party.
9. OTHER MATTERS
Acknowledge other relevant matters relating to the activity/organisation. This may be about your policy in regard to refunding
fees if the member/participant is injured for a long period.
10. ACKNOWLEDGEMENT AND AGREE
Acknowledgement that the individual (this may be a guardian if under 18) has read and agreed to the terms and conditions.

3. MEDICAL TREATMENT
