A 24-year-old bodybuilder presented with painful, enlarging lesions on his buttocks that had developed over the past two months. Physical examination revealed red-brown, infiltrated masses up to 12 cm in size with ulcerations symmetrically on both buttocks. The patient revealed he had been injecting anabolic steroids for bodybuilding purposes over the past 6 months. A skin biopsy showed a foreign body granulomatous reaction with multi-nucleated cells, consistent with intramuscular injections of anabolic steroids.
