– Breathe Easy N Staffs, NHS Stoke-on-Trent & HCPs
– High deprivation, ++ smoking & COPD
• Aim: to integrate a ‘vibrant supportive patient &
carer-led self help group’ into COPD care pathway
– Need-driven developments e.g. new venue,
welcome pack, newsletter, structured programme
– Recognition & referrals/signposting
• Impact:
• 75% are more confident in managing own condition
• 94% have better understanding of their condition Stoke
• 70% have more knowledge of what to do if ill
• Next steps: 22 groups involved. Model now
expanding to EoE. BLF leading next phase in East
Midlands

– Provide a united, loud & respected voice on key policy issues
– Promote consistency & best practice
– Deliver specific work streams yet to be agreed
– Provide an opportunity to engage all key stakeholders
– Complement, not compete with, existing organisations in the
respiratory community
– Be independent of but working with DH/NHSCB
– Become operational over next few months
