How Do I Renew My Coverage?
1. Complete the renewal application you receive from MetroPlus by mail 90
days before your renewal date.
2. You return the application to MetroPlus with updated proof of income via
mail. A premium check must be included with the renewal application, if
applicable.
3. If your information has changed, corrections must be made on the
application and any required proof must be provided or your enrollment may
be delayed.
4. If you need help, call MetroPlus at 1.800.475.METRO (1.800.475.6387) or
TTY 800.881.2812. MetroPlus renewal representatives can arrange to meet
you at a MetroPlus office, your work location or home.
How Do I Pay My Premium?
There are two options to pay your premium:
• Pay My Premium Online
To access online payment, click here: https://www.metroplus.org/paymypremium. This tool
can also be accessed by going to Member Services and then clicking Pay My Premium.
Have your credit or debit card ready. Enter the EXCHANGE NUMBER and INVOICE NUMBER
as shown on your invoice. Then, click ‘Next’ to complete your payment request. (We accept
Mastercard, Visa and Discover cards.)
Please note that the amount shown on the screen reflects the amount of the invoice, not
the amount due for the account.
• Pay My Premium Using a Check
Members that choose not to use online payment can send their Invoice Coupon with a
check to the address listed.
How Do I Find a Doctor or Provider?
• To find a provider, specialist, pharmacy, hospital or other services,
visit http://www.metroplus.org/member-services/find-a-doctor-
provider
How Do I Change my Information or Order an ID
Card?
If you are a member, you can access your account to
• Change your doctor
• Change your address and phone numbers
• Order an ID card
How Do I Find Out What My Benefits Are?
• For help determining your benefits, call Customer Services at
1.800.303.9626 or TTY 800.881.2812.
