What is Your Vision:
What Problem Are You Trying To Solve and WHY? (Remember to include what issues the problem is CAUSING)
What is the actual condition? (current state)
Use the 5 why methodology to better describe current state
What is the Next Target Step?
ent the Change VERIFY Success, & Adjust as Needed
Who is doing what, and when might it be completed? Verify Results - What were your conclusions?
Actual Date Completed: 3/12/2015

1. Why is this problem occurring? There is no guide for
acceptable defects for specific grade surfaces at either press.
2. Why is that?
3. Why is that?
4. Why is that?
5. Why is that?
List Possible Solution(s) -
-create a specific pareto chart for tracking dirt under
-track colors to look for patterns
-find where these colors are stored in the warehouse.
The number one cause of scrap in GRL is particles under the laminate. The parts are often sent
down stream when they should have been scrapped. Because of this behavior, root cause for
this defect cannot be investigated at the point of occurrence.
We send parts that should be scrapped down
stream from our press.
Determine root cause by pareto charting defect
codes at point of defect.
verified that specific high gloss laminates in 4 specific
colors seemed to show defects at 3000% rate
compared to other colors.
revised STD Work to treat these colors differently
and add a quality measure for cleaning underside of
laminat with a brush wand.
