Where the
Opioid
Epidemic
Stands
In 2015, the U.S. led the world in relative and absolute drug related deaths. The
total deaths related to opioid abuse over the last two years in the United States
has taken more lives annually than AIDS did during the height of the U.S. AIDS
crisis in the 1990’s.
▪ According to a Blue Cross Blue Shield Association analysis from 2010 through 2016, the
organization’s covered lives (nearly 30 million in total) showed an increase of nearly
500% of patients being diagnosed with opioid addiction.
▪ During that same period of time, patients from the examined population receiving
medication assisted treatment (“MAT”) increased by only 65%.

Between 59,000 and 65,000 people died from overdoses in the U.S. in 2016,
setting an all time high. The rapid year-over-year rise in overdose related deaths
is directly tied to the growing opioid epidemic. Government agencies, healthcare
providers, and first responders have increased the availability of medication-
assisted-treatment pharmaceuticals (e.g., naloxone and buprenorphine) to help
save lives and aid in the detox and recovery of opioid addicts.

In addition to segmenting treatment options by inpatient or outpatient,
treatment programs can be divided into MAT and drug-free treatments.
Substance abuse treatment that utilizes pharmaceuticals to lessen or
eliminate one’s dependency on a substance is referred to as medication-
assisted-treatment, or MAT.

Common Pharmaceuticals Used In
Medication-Assisted-Treatment
Multiple pharmaceutical treatment options exist for patients who seek MAT programs. Each offer different delivery
methods and require different protocols around administration. The following briefly describes the application and
drug administration differences between the most common medications used in opioid related MAT.
Naloxone
Methadone
Naloxone is used to assist in the detox process for patients. The branded version, Narcan, has received much media attention in the last few years,
as communities have taken advantage of more broadly written prescription laws that allow social workers, first responders, and in some states even
school teachers to carry and administer the drug to save those experiencing an opioid overdose. In addition to infield, acute cases, naloxone is used
to ease the pains and challenges of detoxing in a controlled inpatient clinical setting.
Methadone is a synthetic opioid administered daily to select opioid addicts. It was primarily designed to treat IV heroin addiction. Patients go daily
to a methadone clinic to receive their dosage. The aim is to step down the dosage over time and allow the patient to ween off heroin in a controlled
and measured way. Leveraging research from the Rockefeller Foundation, methadone treatment centers first began proliferating in the mid-1960’s.
Methadone clinics are the most prevalent outpatient MAT facilities in the country. In addition to requiring daily trips to a MAT clinic, methadone can
more easily be abused by addicts to simulate the high they get from other opioids.
Buprenorphine
Naltrexone
Buprenorphine is a commonly used pharmaceutical in the treatment of opioid addiction. It can be administrated to a patient after the patient has
gone through detox. While available in the U.S. since the early 1980’s, buprenorphine become more commonly used in opioid MAT programs as a
result of an early 2000’s effort to train and certify primary care physicians to administer buprenorphine to the growing patient base that sought
opioid addiction treatment. Buprenorphine can be combined with naloxone to create a drug that both helps manage the addiction, while limiting
addiction cravings and limiting the chances for patients to abuse the drug recreationally. The most common branded form of buprenorphine with
naloxone is Suboxone, used in the treatment of opioid addiction. The drug can be administered orally or via an implant.
Naltrexone is a monthly shot used to curb an opioid addict's cravings by blocking the opioid receptors. The most common branded version is
Vivitrol. While different patients respond better to buprenorphine or methadone, some patients respond well to naltrexone. The drug was
originally produced to treat alcohol dependence and the FDA approved it for treatment of opioid addiction in 2010. Many jails and prisons prefer
treating inmates with naltrexone due to the frequency and controlled delivery method. Since 2010, Vivitrol has marketed heavily within this
channel. Questions surrounding efficacy have been recently raised for the use of Vivitrol in treating opioid addiction, with researchers calling into
question the validity of the study used in the FDA application.

Buprenorphine
Growing Usage
Buprenorphine, as a course of treatment for opioid addiction, continues to
expand thanks to regulatory changes and growth in opioid addicts. As the
demand grows for buprenorphine, so does the scrutiny around prices for the
branded version, Suboxone. Last year, 35 states filed suit against Reckitt
Benckiser for price fixing and monopolistic practices related to the spinout of
Suboxone in 2015.

The substance abuse industry has a legacy of fraud and abuse that has been highlighted in the media in recent
years. One of the largest substance abuse treatment companies in the country, American Addiction Centers, has
been improving compliance after fraud charges were levied by regulators relating to their lab business. In 2015, a
documentary—The Business of Recovery—was released highlighting high rates or fraud in “resort” inpatient
facilities and sober living homes.
Due to growing public awareness around the need for substance abuse treatment and the 2008 legislative mandate
that declared mental illness and substance abuse as a pre-existing condition, greater emphasis is being placed on
substance abuse treatment providers to demonstrate efficacy and value. Payors are working with treatment
centers to better track and understand treatment results with the aim of paying for what works. The publicity of
less than above board industry participants has helped further the incentives for legitimate participants of size and
scale to improve their tracking and outcomes reporting.

As of 2017, the rules around prescribing buprenorphine where changed by HHS to expand the number of patients
in a year a certified physician can prescribe from 100 to 275. Payors are actively working with substance abuse
treatment programs to expand the number of clinics to meet the demand for treatment services.

Strategies
Shift Toward
Outpatient
Treatment
The expansion of substance abuse addiction as a pre-existing condition has made payors more sensitive to paying
out-of-network fees. As a result, payors are favoring in-network doctors and facilities.
Inpatient treatment facilities typically range from $450 to $1,500 per patient per day. These facilities aim at first
detoxing patients, then imparting skills and coping mechanisms to manage patients’ addictions. Seeking to control
the costs of treatment, payors increasingly favor short lengths of stay at inpatient treatment and more rapid
enrollment for the less expensive outpatient treatments. Interviews with numerous industry participants noted an
increase in the number of battles providers must endure around getting the appropriate treatment for the patient,
as insurers seek to classify more addiction patients in lower acuity (lower cost) categories.

The total deaths related to opioid abuse over the last two years in the United States has taken more lives annually than AIDS did during the height of the U.S. AIDS crisis in the 1990’s. ▪ According to a Blue Cross Blue Shield Association analysis from 2010 through 2016, the organization’s covered lives (nearly 30 million in total) showed an increase of nearly 500% of patients being diagnosed with opioid addiction. ▪ During that same period of time, patients from the examined population receiving medication assisted treatment (“MAT”) increased by only 65%. Substance abuse treatment that utilizes pharmaceuticals to lessen or eliminate one’s dependency on a substance is referred to as medication- assisted-treatment, or MAT.   • 9. Copyright © 2018 Splash 4 Partners, LLC Common Pharmaceuticals Used In Medication-Assisted-Treatment Multiple pharmaceutical treatment options exist for patients who seek MAT programs. Each offer different delivery methods and require different protocols around administration. The following briefly describes the application and drug administration differences between the most common medications used in opioid related MAT. Naloxone Methadone Naloxone is used to assist in the detox process for patients. The branded version, Narcan, has received much media attention in the last few years, as communities have taken advantage of more broadly written prescription laws that allow social workers, first responders, and in some states even school teachers to carry and administer the drug to save those experiencing an opioid overdose. In addition to infield, acute cases, naloxone is used to ease the pains and challenges of detoxing in a controlled inpatient clinical setting. Methadone is a synthetic opioid administered daily to select opioid addicts. It was primarily designed to treat IV heroin addiction. Patients go daily to a methadone clinic to receive their dosage. The aim is to step down the dosage over time and allow the patient to ween off heroin in a controlled and measured way. Leveraging research from the Rockefeller Foundation, methadone treatment centers first began proliferating in the mid-1960’s. Methadone clinics are the most prevalent outpatient MAT facilities in the country. In addition to requiring daily trips to a MAT clinic, methadone can more easily be abused by addicts to simulate the high they get from other opioids. Buprenorphine Naltrexone Buprenorphine is a commonly used pharmaceutical in the treatment of opioid addiction. It can be administrated to a patient after the patient has gone through detox. While available in the U.S. since the early 1980’s, buprenorphine become more commonly used in opioid MAT programs as a result of an early 2000’s effort to train and certify primary care physicians to administer buprenorphine to the growing patient base that sought opioid addiction treatment. Buprenorphine can be combined with naloxone to create a drug that both helps manage the addiction, while limiting addiction cravings and limiting the chances for patients to abuse the drug recreationally. The most common branded form of buprenorphine with naloxone is Suboxone, used in the treatment of opioid addiction. The drug can be administered orally or via an implant. Naltrexone is a monthly shot used to curb an opioid addict's cravings by blocking the opioid receptors. The most common branded version is Vivitrol. While different patients respond better to buprenorphine or methadone, some patients respond well to naltrexone. The drug was originally produced to treat alcohol dependence and the FDA approved it for treatment of opioid addiction in 2010. Many jails and prisons prefer treating inmates with naltrexone due to the frequency and controlled delivery method. Since 2010, Vivitrol has marketed heavily within this channel. Questions surrounding efficacy have been recently raised for the use of Vivitrol in treating opioid addiction, with researchers calling into question the validity of the study used in the FDA application. As the demand grows for buprenorphine, so does the scrutiny around prices for the branded version, Suboxone. Last year, 35 states filed suit against Reckitt Benckiser for price fixing and monopolistic practices related to the spinout of Suboxone in 2015. The total deaths related to opioid abuse over the last two years in the United States has taken more lives annually than AIDS did during the height of the U.S. AIDS crisis in the 1990’s. ▪ According to a Blue Cross Blue Shield Association analysis from 2010 through 2016, the organization’s covered lives (nearly 30 million in total) showed an increase of nearly 500% of patients being diagnosed with opioid addiction. ▪ During that same period of time, patients from the examined population receiving MAT increased by only 65%. Copyright © 2018 Splash 4 Partners, LLC Select Macro Substance Abuse Trends Misconduct & Fraud Shift Toward Evidence Based Treatment Plans The substance abuse industry has a legacy of fraud and abuse that has been highlighted in the media in recent years. One of the largest substance abuse treatment companies in the country, American Addiction Centers, has been improving compliance after fraud charges were levied by regulators relating to their lab business. In 2015, a documentary—The Business of Recovery—was released highlighting high rates or fraud in “resort” inpatient facilities and sober living homes. Due to growing public awareness around the need for substance abuse treatment and the 2008 legislative mandate that declared mental illness and substance abuse as a pre-existing condition, greater emphasis is being placed on substance abuse treatment providers to demonstrate efficacy and value. Payors are working with treatment centers to better track and understand treatment results with the aim of paying for what works. The publicity of less than above board industry participants has helped further the incentives for legitimate participants of size and scale to improve their tracking and outcomes reporting. Expanding Access to Care As of 2017, the rules around prescribing buprenorphine where changed by HHS to expand the number of patients in a year a certified physician can prescribe from 100 to 275. Payors are actively working with substance abuse treatment programs to expand the number of clinics to meet the demand for treatment services. In-Network Strategies Shift Toward Outpatient Treatment The expansion of substance abuse addiction as a pre-existing condition has made payors more sensitive to paying out-of-network fees. As a result, payors are favoring in-network doctors and facilities. Inpatient treatment facilities typically range from $450 to $1,500 per patient per day. These facilities aim at first detoxing patients, then imparting skills and coping mechanisms to manage patients’ addictions. Seeking to control the costs of treatment, payors increasingly favor short lengths of stay at inpatient treatment and more rapid enrollment for the less expensive outpatient treatments. Interviews with numerous industry participants noted an increase in the number of battles providers must endure around getting the appropriate treatment for the patient, as insurers seek to classify more addiction patients in lower acuity (lower cost) categories.
