INCIDENT DETAILS: Ventilators were found on the ground at WH10
PROJECT : Replacing new roof and gutter for WH9 and WH10 (EMEPMI)
DATE: 2 / 1 / 2016 TIME: 8.30AM LOCATION: WH9 & wh10
Incident Description: Ventilators were found at the tent area in EMEPMI compound at
Saturday morning on 2 January, 2016. It was found at the ground on Saturday morning
because on Friday , there were no findings on falling ventilators.
There were Two possibilities that could lead to this incident:
1. Blown by the strong wind condition.
- Some of the ventilators were stack together and some of it were screwed to the old
roof. Investigation found that probably the stack ventilators fall down because of the
strong wind. Finding also it is difficult that the ventilators can travel that far given that
there is a gutter in the middle of the roof.
It should have stuck at the gutter by looking at the body of the ventilators, it is rectangle
shape and couldn’t drop into the middle gutter and simply roll back up to the end roof to
fall down at the tent area. Thinking of logic, wind should come from the front of the
2. picture which is from the sea. It shouldn’t fall down at the tent area of EMEPMI. But
then, it is not our power to question God’s power. Anything could happened.
2. No securities at the monkey ladder
- at the back of the warehouse which near the area of the berth, located the monkey ladder
leads to the roof. The ladder padlock were leave open all the time for future access.
Anyone could climb up the ladder and simply throw the ventilators down at the tent area.
We are not accusing anyone for this incident but this is a possibilities that we could
accounted for.

CORECTIVE ACTIONS
We are bringing down all the ventilators and placed it in the EMEPMI yard. As for now
74 ventilators has been brought down from the roof. We bring down the ventilators that
we dissembled only which is 3 or 4 ventilators everyday when the roofing work is in
progress.
