 Conducted MDS assessments on residents on admission, significant change,annually and
quarterly and established care plans for long-term residents.
 Facilitated otherrecreational groups such a games, puzzles, crafts, etc.

Hudson Valley Hospice, Poughkeepsie, NY
 Managed a caseload of 16-18 individual hospice patients in nursing home, homecare and hospital
inpatient settings for 40 hours/week.
 Produced an initial assessment report on each new music therapy patient and documented each
further visit using a specialized progress note.
 Planned and facilitated appropriate music therapy interventions for a children’s bereavement
group every other week.
 Participated in interdisciplinary team meetings including doctors,nurses,social workers, and the
volunteer coordinator every week.
 Worked as part of an interdisciplinary team in the field, co-treating with chaplains, nurses,
