DJR
Conduct and document an initial evaluation of trainee qualifications within 60 days
of initial assignment (120 days for ARC), either PCS or PCA. Document the evaluation
on AF Form 623a or automated version and file in the OJT record. As a minimum
the evaluation should include:
1. AFSC, duty position, core and HST, and deployment/UTC requirements.
2. CDC requirements. Note: 30 days per volume.
3. Formal/informal training requirements, such as skills, knowledge, or classroom instruction. Supervisors must evaluate technical
school graduates on all tasks taught in the initial skills course as identified in the STS portion of the CFETP.
4. Other local or unique training requirements.
5. Supervisor and trainee responsibilities as outlined in this AFI and other governing directives, such as AFI 36-2101 and the CFETP.
6. Supervisors are encouraged to allow trainees to complete course work during normal duty hours, if mission requirements allow.
Supervisors will monitor trainee progress to ensure timely completion of CDCs or ADL course work.

DJR
IAW AFI 36-2201, Chapter 6, paragraph 6.10., Table 6.1., and Attachment 10:
I have issued the CDC materials to the supervisor and trainee.
An inventory was conducted. All materials were accounted for and the edit code was verified as correct. The trainee, with assistance
from the supervisor, must make course corrections and pen/ink changes as needed. The supervisor will determine the CDC volume
sequence. The trainee will have no longer than 30 days per volume. If duty permits, time can be given to complete CDCs. However, it
is the trainee's responsibility to allocate off-duty time to complete and study CDC materials. The trainee must complete all Unit Review
Exercises (URE) and Self-Test questions. Once the volume is completed, the URE questions must be transferred to the "Field Scoring
Sheet". The supervisor will review missed questions and document the bottom of answer sheet prior to issuing the next volume.
