The laparoscopic approach has been adopted in the last years in the majority of surgical
disciplines, including urology. Its main advantages are the amplification of images and the
reduced invasiveness.
The 3D vision is a new technology which has been more recently coupled with both
laparoscopic and robotic approaches.
The main disadvantage of this technology is the discomfort created to the surgeon which
appears to be more evident following long lasting surgical procedures. This disconfort is the
result of an altered visual perception which has not been untill now fully elucidated and which
is attributed to the use of polarized lenses by the surgeon.
The first aim of our research was to document the presence of a visual alteration following a
series of laparoscopic procedures using the 3D with polarized glasses compared to the 2D
technology. The second aim was to introduce in clinical practice a new 3D Realvision monitor
which avoids the need of polarized lenses.
A severe alteration at the orthoptic tests was invariably documented in all members of
surgical teams following the use of 3D monitors with polarized glasses in laparoscopic
procedures.

On the other hand, no abnormalities of conventional eye exam and of the visual evoked
potentials were evident post operatively following the utilization of 3D Realvision monitor and
other ordinary 2D tools.
When RealVision 3D System was employed for a surgical simulation in the pig, results were
fully satisfactory according to the statements of 3 surgical teams.
RealVision 3D monitor was subsequently employed in humans in a randomized controlled trial.
The aim of this trial was to test efficiency and invasiveness of the monitor with respect to
conventional lenses during a homogeneous series of 3D laparoscopic procedures.
Ad interim results indicate that the use of RealVision 3D monitor is satisfactory and avoids
all visual abnormalities which are detected at the orthoptic tests in all surgical teams after
conventional 3D with polarized glasses laparoscopy.

5Executive summary
RealVision 3D System consists of hardware and software that are protected
by worldwide patents.
Without the obstacle of a polarized barrier, coloured filters and polarized filters,RealVision
3D System grants a “natural” vision as pupil does not dilate and, consequently,
accommodation and relative convergence are not altered.
At cortical level, RealVision 3D System produces a visual experience (both real and
constructive) where the visual conditions are the patient’s natural ones. Therefore it allows
a more truthful and careful measurement of the patient’s condition.
An ordinary 3D system with polarized glasses may frequently cause troubles to sight.
Instead, RealVision 3D System does not produce any trouble to sight and everyone can
see in 3D (except people with poor visual ability).
Software included in RealVision 3D System has many innovative features and
characteristics that have allowed to patent it.
One of these is the possibility to create an “inlet filter”: RealVision 3D System is able to
convert several system/software languages - produced by different sources (incoming and
outgoing) - into a language compatible with its wireless devices.

6Executive summary
This novelty allows to avoid modification and/or installation of different software
applications on existing (for example, hospital) devices, making RealVision 3D System
compatible with a great economy of expenditure and installation times.
Furthermore, RealVision 3D System avoids additional training of operators and
maintenance and allows updating of tools/machinery currently installed, making said
equipment more modern and efficient.
Another important feature of RealVision 3D System is the possibility to stream wi-fi
between microscopes or 3D cameras without slowdowns or deferred storms that would
vitiate a surgery laparoscopy. RealVision 3D System will allow, for example, a specialist in
New York to watch live and in 3D a surgery taking place in Rome (today not possible given
the unbearable “weight” of a stereoscopic 3D video).

Worldwide, there are well-known TV brands (such as Sony, LG and Samsung) that offer
- at very high costs - “last generation” video solutions which include 3D viewing without
glasses.
RealVision 3D System is substantially different.
As a matter of fact, other systems provide a “visual illusion“ created via a software that
simulates a 3D through the superposition of different images with different depths, with
the result of perceiving an unrealistic effect that is fatiguing to the human eye.
RealVision 3D System is the only real 3D that combines hardware and software to create a
