Background and AimBackground and Aim
Mouth rehabilitation of edentulous Angle Class III
patients is aimed at achieving a Class I key relation
and normal overbite and over jet, regardless of the
position of the maxilla and mandible . The relation-
ship between the jaws contraindicates the cons-
truction of fixed prostheses (bridges) over implants
and there is also the crucial problem of restoring
facial aesthetics. An overdenture is often conside-
red the proper treatment of choice for patients with
moderate to severe bone resorption, problems with
dexterity and phonetic problems due to palatal
contours. The use of an overdenture on implants
and the securing of its stability by the conical crown
system SYNCONE , ANKYLOS FRIADENT compo-
nents, is the goal of our case presentation. It has
been shown that the SynCone conical crown system
could potentially provide adequate and constant
retentive force to retain implant supported over-
dentures.
Two maxillary edentulous patients presented
themselves at our clinic, asking for a treatment
based on implants to replace the missing teeth and
solve aesthetic problems. Clinical and radiogra-
phic examination indicated skeletal malocclusion
Angle Class III.
Both were suggested to be treated by hybrid
prostheses fixed on implants (short without palatal
plate overdentures), well stabilized on SYNCONE
ANKYLOS telescopic crowns, precise prefabricated
components. The above malocclusion was treated
by acrylic teeth arranged in normal articulation and
the use of labial, like full denture’s, acrylic flanges,
improved their aesthetic outlook. Both patients had
various bridge constructions in order to move their
new teeth labially and the long term outcome was to
lose their upper teeth.
In case 1, a 61 year old female, the maxillary
deformity destroyed any sign of her self- esteem
and being a manager in an insurance company, she
demanded the best aesthetic result. She was
treated with 6 ANKYLOS implants and an on
Patients with Angle Class III malocclusion face
both functional and aesthetic problems as well as
psychological and social ones. The correction of
the malocclusion and the improvement of their
aesthetics allow them to recover their self-esteem.
Orthognathic surgery can treat such deformities
and solve functional and aesthetic problems, but
the following postoperative discomfort forces
older patients to ask for an implant solution.

The use of implants allowed us to construct stable
prostheses with normal teeth articulation and facial
improvement. The SYNCONE system provided us
with the proper interarch space to be able to fix their
artificial teeth in a position of normal articulation, as
Angle Class I. Our patients were very satisfied with
this treatment and they loved their new facial
appearance.
Solving articulation irregularities and facial
impairments can provide our Angle Class III
edentulous patients with the necessary conditions
for recovering their self-esteem . Patient
satisfaction is highlighted in many studies,
alleging the benefits gained due to the change in
appearance, which reflect a patient’s desire to
recover self-esteem, among other aspects.

SYNCONE overdenture. In case 2, the 58 year
old male, being a singer had lost his upper teeth
after many attempts to have better frontal teeth
appearance. He was also treated with 5 ANKY-
LOS implants and a SYNCONE overdenture. In
both cases the patients were informed about the
surgical correction of the maxilla and neither
decided to have it. The study model analysis and
the following waxing process gave them the
opportunity to reach their decision.
Postoperatively, the patients wore a
full denture in the desired position and
occlusion. They had the time to express any
problem and their experience to the new
situation, before having a final copy by a
special key.
