4) Assisting therapists by following patients in wheelchairs during walks, changing wheelchairs or cushions, and monitoring patients on equipment when therapists need to attend to other patients.
2. Working with spread sheets
We work closely with spread sheets. This is continuous, as we enter Doctors
orders as they arrive, enter plans of cares, and daily therapy treatments.
3. Entering documentation on the spread sheet
When receiving a doctor’s
order
On the spread sheet, locate the
patients name, and the correct
discipline, under date certification
was signed, place the month and
date and hit enter. Many times all 3
disciplines are on the order,
sometimes only one is on there.
When receiving a signed Plan
of Care or an Undated plan of
care.
On the spread sheet, locate the
patients name, under signed P.O.C.,
or U.P.O.C. Check to see what
discipline the care plan is for, enter
the month and date on the signed
P.O.C., or U.P.O.C. then press
enter. Many times all 3 disciplines
are returned together, sometimes
they are not.
Entering therapy notes
When receiving a therapy note,
make sure a medical necessity is
mentioned, check for unfinished
sentences, or mixed wording or
misspelled words. On the spread
sheet, locate the patients name,
and make sure your in the right
discipline the note is on, then enter
the month and date the note was
written for that patient.
4. Proofreaders
Techs also have the responsibility of proof reading therapy notes. Looking for missing
notes, miss phased sentences, or missing treatment codes. Insurance companies will
try to get out of paying based on a technicality.
5. Sanitize the therapy gym
We must make sure therapy beds, stationery bikes, and other equipment is sanitized daily.
We treat many patients daily, some with compromised health issues, so keeping the gym
clean and germ free is a must.
6. Sanitizing the therapy gym
Sanitizing the therapy beds.
Get a wash cloth, put on protective
gloves and get the cleaning
solution, which is locked in the
bathroom cabinet. The key is
located on the outside ledge of the
cabinet. Spray the therapy beds
and wipe down. When dry place a
new sheet on the bed, and replace
pillows. If pillows have no cases on
them, you will have to place new
ones on them.
Sanitizing gym equipment
Spray cleaning solution on the wash
cloth and wipe down the parallel
bars, and stair rails. Also wipe
down the handles on stationery
bikes.
Sanitizing other equipment
It is important to sanitize other
equipment such as cones, pegs, and
gait belts. For cones and pegs, lay a
towel down on a table. Place pegs
on the towel and spray the cleaning
solution on them. Then spray the
inside of the peg container, wipe
the container out, then wipe the
pegs off and place back in the
container. Do not for get to spray
and wipe off the peg board. The
cones can be laid on the towel and
sprayed down and then wiped off.
Gait belts are wiped down with a
wash cloth that has been sprayed
with cleaning solution.
7. Filing
We do a lot of filing in office, and in patients medical charts. Some files are
kept 6 months, some for 7 years, so we are always purging.
8. Where to file, and what?
Telephone orders
You need to make copy of the
telephone order, one for the in
office files, and one for the hard
chart upstairs. When filing upstairs,
make sure you are under Physicians
orders when filing the telephone
order in the patients chart.
Plans of cares, Updated Plans
of cares.
Make a copy, one goes in the in
office patient file, under the
discipline it is for. For instance, if it
is a speech therapy plan of care,
James Jones, then it goes in the
speech therapy folder for James
Jones. In the hard chart, it gets
filed under Rehab notes. Look
towards the back section. Physical
therapy is in front of the rehab
section, occupational is in the
middle, and speech is last.
Therapist notes
Therapists notes only need to go in
the patients hard chart. If it is a
occupational note, place in the
occupational section of the hard
chart. Newest notes first!
9. Assisting patients out of bed
Many times when we are asked to get a patient for therapy, we assist
the patient with a bed to chair transfer.
10. When returning patients to their rooms.
• Remember to always attach the call button to the patient.
• Put their side table close to them, and make sure they are able to each their phone,
water, and other requested items.
• If they request medication, or make other requests that are handled by other staff, be
sure to alert the nurse or C.N.A.
It is our responsibility to make sure needs are met, that proper staff are alerted for special
requests, and that every thing is accessible to the patient once you leave the room.
11. Customer Service
We assist patients to and from their rooms, and assist therapists with treatments. So
good customer service skills is a must.
12. Assisting therapist
Chair follow
This is probably the most common
assist of an aide. A therapist
walking a patient, may ask you to
follow the patient with their wheel
chair, while the therapists is
guiding them with their walk. This
is in case the patients legs give
out, or they get they get tired.
Most of the times the therapist has
the patient walking in the gym
area, or sometimes in the parallel
bars.
Changing wheelchairs, or a
chair cushion
Another way to assist therapists is
when they need to switch out a
chair, or cushion. They will be
assisting a patient with a stand,
and you quickly will switch out the
equipment they are exchanging at
that time.
Stationery bike, or other
equipment
Many times the therapists has to go
onto another patient, and their
time has ran out on their current
patient. You may be asked to sit
with a patient who is finishing their
time on a stationery bike, or they
make be on therapy machine. When
their is up, just unhook and take
them back to their room.
