Dear Ms. Quiroz:
I would like to take this opportunity to thank you and Mr. Ahmed Gamea, my
son's Medicaid Scivice Coordinator, for your help in obtaininga car seat lift to transport
om son, Ali Ahmed. We started the application process over a year ago, and two other
service coordinators came and went, but Mr. Gamea stayed on the case until the lift was
finally installed in our van. He took overmy son's case last fall and helped us assemble
the paperwork so that the application packet was completed according to state
requirements. He then followed up with both OPWDD and the vendor in a timely and
professional manner, truly an example ofoutstanding service. We are grateful for Mr.
Gamea's assistance and pleased with the new car seat which will enable us to bring Ali to
appointments and on excursions so much more safely and easily.
Thank you for supporting my son's case with your selection and
supervision ofhis service coordinator. We look forward to continuing our relationship
with Mr. Gamea and Eihab Human Services as our son grows older.
