MID-YEAR REPORT
Memorial is required to provide
progress reports on the programs we
fund periodically during the year, from
health system reporting to President
reports and the IRS.
Providing us with a mid-term project
report allows us to communicate
program status, keeps organizations
accountable for the funds they were
granted and helps to prep
documentation for year-end reporting.
As a bonus, it gives our partners a
opportunity to showcase their success
and ask for help with any problems or
concerns they may have.

MID-YEAR REPORT
ANNUAL REPORT
is an opportunity to present outcomes
at the successful completion of the
program. Analyzing success and
shortfall helps to adjust program and
project trajectory, focusing on not just
the needs of the community, but also its
capacity for change.
Not every project is systemic; not all are
designed to be. However, with
measurement and outcome analysis
every program can inform the greater
goal of a healthier community. Positive
or negative results are an important
indicator of current community
support, opportunity and limitations.

9. BUDGET
Obviously, the budget is a vitally
important part of any organization’s
proposal. Requesting funds is a great
deal more difficult if the required
amount is unknown.
The budget should provide a clear
overview of what everything is
expected to cost and who/how that
cost will be covered.
Memorial has clear regulations on what
we can provide funding for, as the
template clearly indicates. However,
we need to know that the costs we
cannot cover will be provided by
someone, or the program’s chance of
success is sabotaged from the start.

10. BUDGET
INVOICE
Memorial understands that our
partners’ time is limited. In an effort to
alleviate some additional stress, we
developed a standard invoice which
can be used to request awarded
funding at the agreed upon intervals.
We require an invoice from the
receiving organization to facilitate
check release. This template includes
all the information we require to
process and release funds.
Organizations are always welcome to
utilize invoices of their own design
provided it includes all relevant
information.

and meet requirements we need to
provide to both our board and

programs we fund, but also to provide
accountability to the hospital and
