This document provides instructions for teachers at Eastview High School on how to complete and submit online behavioral referrals using their student information system, Infinite Campus. Key steps include selecting the behavioral referral form from the index, filling out required fields like the date, time, location and details of the incident, adding the involved student as a participant, and submitting the draft for an administrator to review and resolve. The online system allows referrals to be easily created, edited, and sent directly to administrators without using paper.
2. ONLINE BEHAVIOR REFERRALS
Ease of access – always available online
Editable – can write/re-write/spell-check/etc.
Send directly to Admin – completed form goes
directly into Admin queue
Green – no paper!
3. GETTING STARTED…
Click on INDEX tab in the left-side menu
Under BEAHVIOR, select BEHAVIORAL REFERRAL.
Click NEW.
4. COMPLETING THE FORM
Start with the required fields in RED at the top of
the referral
ALIGNMENT defaults correctly to Discipline (no need
to change)
The DATE of INCIDENT should be the actual date the
even took place (it will default to the current date, so
be sure to back date if referral is being submitted on a
later date)
The TIME of INCIDENT should be a close estimate

Select the Administrator to NOTIFY

Format for creating event title:
2 or 3-word description of event

CONTEXT:
Choose During School Hours (7:30am – 2:25pm) or Outside
School Hours from dropdown menu
CONTEXT DESCRIPTION:
Additional helpful details about when the incident took
place
Examples: p5 lunch, at football game, passing time
LOCATION:
Choose from dropdown menu
LOCATION DESCRIPTION:
Additional helpful details about where the incident took
place
Examples: B3 locker bay, weight room, A1 boys bathroom

DETAILS:
This is the narrative of the incident, from your
perspective
Keep it relatively brief (think Twitter), giving enough
information to accurately describe the incident
Keep in mind that this information will be seen by
other staff (admin/SRO/counselor/case manager/etc.),
parents and student
Remember that you cannot use any other student
names in this section (data privacy laws)

ADD EVENT/PARTICIPANT:
Click the button at the bottom of screen

10. COMPLETING THE FORM
Leave WEAPON blank
Go to PARTICIPANT DETAILS
Leave the FILTER as STUDENTS and type in the
name (partial or full) of the student; click SEARCH
Leave the rest of this section blank
ADD PARTICIPANT if more than one student was
involved with the same situation (the event details
will remain the same, so if this student’s role was
different, complete a separate referral)
Click SAVE DRAFT
11. REVIEW THE INFORMATION & SUBMIT
Review the information on the screen and double-
check for accuracy
Make changes if
necessary
Click SUBMIT
12. WHAT HAPPENS NEXT?
The referral will automatically go into Kim or Jodi’s
queue
As we take meet with student and assign a
resolution, we will enter information into Campus
to complete the referral and display the incident
and resolution on the portal
