2. A study done to understand why readmission rates
were high in psychiatric patients by the US
Department of Health and Human Services stated:
Several factors drive these high readmission rates,
including inadequate discharge planning, medication non-
adherence, lack of systems to ensure outpatient behavioral
health follow-up, impairment to patients' ability to care for
themselves, lack of access to community services and
family support, and substance abuse.
