It is a documentation issue. The clinician’s documentation must contains the information necessary to have a very specific Diagnosis. Only then would we be able to produce a specific ICD-10 Code. 2- We look at 4 common ICD-10 implementations approaches. These approaches had some advantages and disadvantages. Tread carefully when selecting vendors for your ICD-10 solution. Ask the right questions. 3 - T-System’s ICD-10 implementation addresses the clinical documentation requirements for ICD10 specificity.
