Who do we support?
PHASE 1
2015:
•	 Establish a co-ordinated effort and
	 promote more internal collaboration
•	 Begin to build capacity of housing
	 communities (local goals and
	 community plans)
PHASE 2
2016:
•		Leverage resources to expand the
		availability of social and personal
		supports
•		Further develop and formalize external
		partnerships
The Community Health Framework marks the beginning of the conversation
around community health. As the Region moves forward with framework
implementation, it is important to keep the conversation going.

What we heard
The Vision:
To initiate a process aimed at improving the
physical and social environments of residents
and more effectively using community resources
and supports to enable people to achieve housing
stability and cope with challenges that impact
their well-being.
Principles:Key directions:
• Establish a co-ordinated effort
• Promote more internal collaborations
• Build capacity of housing communities
• Create awareness and connections to support services
• Developing resources to expand the availability
of social and personal supports
• Further leverage partnerships

Framework?
An initiative aimed at improving the physical
and social environments of our clients,
and more effectively using community
resources and supports to enable people to
achieve housing stability and cope with the
challenges that impact their well-being.
This approach:
• Creates healthy supportive environments
and improves access to social supports
and services
• Emphasizes community participation and
develops community capacity
• Involves co-ordination, collaboration, and
partnerships with a variety of sectors
• Focuses on supporting vulnerable groups
