2. What Happened
On a particular date, (…) a fire broke out in a given location in a
company. One of the kitchen staff attempted to put out the fire and
sustained 40% burns. The fire set off the detectors leading to the
sounding of the fire alarm. All personnel within the building were safely
evacuated to the respective fire assembly points and the injured person
was picked up by the ambulance and rushed to hospital while being
given first response care by the clinic team
3. Learnings
• There was confusion amongst visitors on how to respond and where to
assemble
• The casualty was poorly handled due to the lack of a stretcher to carry the
injured personnel to the ambulance.
• The evacuation response from staff was noted to be slow with reluctance to
respond both to the emergency and the search and rescue operation that
subsequently followed.
• There were no trained first aiders to administer initial first aid care as the
medical response team were awaited to arrive in the ambulance.
• Staff and visitors responded without masks and the 1.5m social distancing
rule was not observed within the assembly points
4. Corrective and Preventive
Actions
• Always have your PPE close by and always have your mask on you.
• Social distancing in any gathering must be strictly adhered to for your safety and that
of your neighbor.
• All access and egress into the KPC facilities must be halted as soon as the emergency
alarms sound to allow for movement of emergency vehicles and safe evacuation of
personnel.
• The medical standby team must be ready to respond to emergencies within a shorter
time frame to minimize the chances of a fatality.
• All staff must respond promptly to an alarm for your own safety
• Security and safety teams must be better prepared to shuttle evacuees to the right
assembly points and update the evacuees as information is received from the
response team
