INTRODUCCION :
La enfermedad de Lou Gehrig o esclerosis lateral amiotrófica (ELA) en los países anglosajones, también conocida como enfermedad de la motoneurona, es una enfermedad neurodegenerativa mortal sin tratamiento eficaz conocido. Esta enfermedad causa una parálisis progresiva debido a la degeneración de las neuronas motoras tanto superiores como inferiores.
La ELA es una enfermedad poco frecuente, que afecta a más hombre que mujeres. Aparece en la edad adulta, siendo el promedio de edad 55 años. Además, se sabe que del 5% al 10 % de los pacientes tienen antecedentes familiares de ELA. (1)
Existen casos extraordinarios como el de Stephen Hawking que lleva padeciendo la enfermedad desde 1963. Esta enfermedad conduce inevitablemente a la muerte, que suele ocurrir de 2 a 5 años después del diagnostico por problemas respiratorios, ya que estos músculos dejan de funcionar (2).
OBJETIVO:
Definir las terapias físicas, evaluar los resultados, establecer las más apropiadas y dar a conocer el valor neurorrehabilitador de las mismas.
MATERIAL Y METODOS:
Revisión bibliográfica de artículos publicados en PEDro, Cochrane, Medline y Pubmed, que refieren terapias físicas y respiratorias como herramienta neurorrehabilitación.
RESULTADOS:
La terapia física comprende trabajo cardiovascular (3,4,5,6), ejercicios de fuerza (7) y flexibilidad(8), específicos respiratorios(2), resultando beneficiosa por la reducción de las condiciones que amenazan la vida. El tratamiento farmacológico con Rizulole (9) aparece siempre como complemento a las terapias físicas, junto a masaje (9) y otras técnicas fisioterápicas para paliar síntomas (10) en los estadios mas avanzados.
DISCUSION:
La fisioterapia respiratoria junto a la ventilación mecánica (2) en los casos indicados y el masaje contra el dolor y la espasticidad (9), deben formar parte de la terapia física en la ELA.
La fisioterapia debe aportar al equipo interdisciplinar diferentes técnicas (8,10,11), como ejercicios de acondicionamiento físico (3,4,5,6,7), específicos respiratorios y de diafragma (8,10), afectados invariablemente en la ELA (2).
Por lo que el equipo interdisciplinar debe manejar dichas terapias como cuidados paliativos, junto a la colaboración en las mismas por parte de la familia y el propio paciente para el mantenimiento de la calidad de vida (2,8,10,11)
CONCLUSION:
La eficacia del ejercicio como terapia física ha demostrado en varios estudios ser neuroprotector tanto del SNC como del SNP, además de estimular al sistema inmune y sistema cardiovascular. Se discute si el ejercicio intenso es perjudicial o no. Serán necesarios más estudios para comprobar el efecto de las demás técnicas fisioterápicas como tratamiento neurorrehabilitador.
