VARICOCELE

-A varicocele is an abnormal dilation of the
veins of the pampiniform venous plexus in the
scrotum.
-Varicoceles usually occur in the veins on the
upper portion of the left testicle in adults.
-In some men, a varicocele has been associated
with infertility.
-Symptomatic varicocele
(pain, tenderness, and discomfort in the
inguinal region)
-is corrected surgically by ligating the external
spermatic vein at the inguinal area.
-An ice pack may be applied to the scrotum for
the first few hours after surgery to relieve
edema.
-The patient then wears a scrotal supporter.
VARICOCELECTOMY
-A varicocelectomy is the high ligation of the
gonadal veins of the testes.
-Varicocelectomy is done to reduce venous
backflow of blood into the venous plexus
around the testes and to improve
spermatogenesis.
PROCEDURAL CONSIDERATIONS.
-The setup for inguinal varicocelectomy is as
described for an inguinal hernia repair.
-A microscope may be employed to better
visualize the vessels involved.
OPERATIVE PROCEDURE
-The incision may be through a supra inguinal
approach or an oblique inguinal approach
over the external inguinal ring.
 1. The surgeon identifies the structures of the
spermatic cord and dissects the vessels free
from the vas deferens.
2. The abnormal dilated veins in the inguinal
canal are clamped and ligated.
-The redundant portions are excised.
-A drain may be placed.
3. The incision is closed in layers.
