一名47歲婦女日前因急性骨盆腔發炎於他院就診，強效抗生素連用10天仍無法控制感染、高燒不退，傳統治療是以剖腹引流，且由於腹腔腸沾黏極為嚴重，腸子覆蓋在子宮及卵巢膿瘍處，若要切除，稍一不慎即可能弄破腸子，必須做人工肛門。病患轉至臺北醫學大學附設醫院，婦科達文西手術團隊順利切除腸沾黏、子宮及膿瘍，成功突破以往微創手術較難處理嚴重沾黏的限制。北醫附醫婦產部劉偉民主任解釋，以往腹腔沾黏被視為腹腔鏡等微創手術的禁忌，病患若有急性感染發炎、子宮肌腺症、子宮內膜異位、癌症、曾經開過腹腔手術等狀況，均較適合以傳統剖腹處理，以此次收治的病患為例，病患曾接受剖腹產，7年前切除子宮肌瘤，都是容易造成腹腔腸沾黏的狀況。劉偉民主任表示，病患7月初因持續腹痛、高燒不退在他院就診，經診斷是急性骨盆腔發炎，細菌擴及雙輸卵管、卵巢，形成各10公分膿瘍，且子宮肌瘤、腺瘤復發，約10公分大小。儘管嘗試多種抗生素，從第一線、第二線換至第三線的強效抗生素仍舊無法控制感染，若再不把膿瘍引流出來，即可能引發敗血性休克，甚至死亡。原本醫師評估須剖腹進入腹腔引流雙側的巨大膿瘍，才能有效控制感染，且時值急性感染期，腸子黏附在膿瘍四周，腫脹脆弱、不易剝離，難以切除子宮、卵巢、輸卵管等器官。劉偉民主任指出，病患7月中旬轉至北醫附醫接受達文西手術，術中大、小腸全部覆蓋在子宮及膿瘍上，動彈不得，腹腔腸沾黏極為嚴重，藉由達文西的特性如立體視覺效果、顯微手術12倍的放大功能、仿手腕的270度旋轉器械、高級攝影機的自動伸縮功能，最後成功切除所有腸道沾黏，也順利切除子宮、肌瘤及膿瘍，一次解決病患的所有問題。
