This document outlines the CARE model and credentialing process for faculty appointments at a medical institution. It discusses the different types of faculty appointments including clinical, administrative, research, education, and adjunct positions. It then describes the three steps credentialing process: 1) appointment, 2) insurance enrollment, and 3) hospital privileges. Several hospitals are listed where privileges can be obtained. The credentialing process and requirements are explained for both full appointments and lighter "adjunct" appointments. A recruitment and hiring process flow chart is also included, outlining the steps from application to offering a position. Plans for streamlining the process through developing an application and onboarding checklist are mentioned.
