I'm Nathaniel, the Nursing Informatics Specialist and Project Manager at Vila Prosperity.
Recently, we held a meeting with five key stakeholders to evaluate our current health
information system and explore opportunities for improvement. Today, I'll summarize the key
insights from that session and discuss the upcoming enhancements to our system.
At Vila Prosperity, my role involves overseeing the enhancement of our information system to
improve healthcare accessibility, alleviate the workload on our healthcare professionals, and
ultimately enhance health outcomes. The proposed project will unfold over the next five to six
months, encompassing training sessions, pilot testing, and collaborative meetings.
External factors such as an aging population, increasing prevalence of chronic diseases, and
the ongoing Coronavirus pandemic have underscored the need for Vila Prosperity to evolve and

environment, addressing the evolving landscape of healthcare.

Assessment of Current and Desired State of the Information System:
Stakeholders highlighted concerns about the limitations of our current health information
system, particularly in patient tracking and communication. While the system has improved cost
management and reduced medication errors, there's a need for enhancements to ensure
seamless patient tracking and communication channels.
Identification and Definition of Risks:
The absence of remote patient monitoring (RPM) and patient portals emerged as significant
challenges, especially during the Coronavirus pandemic. The lack of RPM affects continuity of
patient care, particularly for those with chronic conditions. Rural areas face additional
challenges due to limited access to convenient healthcare.
Definition of Best Practices for Information System Users:
Recent studies advocate for the adoption of RPM, especially during crises like the Coronavirus
pandemic, to ensure continuity of care and improve patient outcomes. RPM can bridge gaps
between physical check-ups, particularly for patients with chronic conditions such as diabetes.
Patient portals also enhance self-management and patient satisfaction.
Utilization of Technology:
Stakeholders unanimously agreed on the importance of integrating RPM applications on
computers and mobile devices, along with ensuring easy access to patient portals. These
technologies are expected to streamline workflows, enhance communication, and improve data
capture for both providers and patients.
Workflow and Communication:
RPM technologies are anticipated to enhance workflows by providing continuous patient support
and connectivity. Personalized messages based on patient profiles will offer reassurance, while
streamlined data collection will improve process efficiency and communication.
Data Capture:
Centralized data storage through patient portals will save time for providers and patients alike,
eliminating the need to navigate multiple systems.
Practice and Outcomes:
RPM represents a promising approach, particularly given the aging population and increasing
health challenges. Advanced technologies enable remote patient monitoring, allowing
individuals to receive care without disrupting their daily routines. This approach benefits patients
with chronic conditions, elderly individuals, and preterm infants by offering contactless
monitoring and ensuring continuous care.
