Medicare Physicals
Question: I would like to know more about
coding a medicare annual & subsequent visit
comparing to a regular physical which we all
know medicare doesn't cover.
They cover 1 welcome to medicare physical &
then starts with the annual or subsequent visits.
ANSWER
Question: In May’s Webinar you referenced a CLIA
WAIVED modifier - QW, that was linked to a Rapid
Strep Test. How did you know to attach the QW?
I am not even certain what it means.

Question: I am having trouble putting it all
together! Reading a question and figuring out
what to do and cardio issues.
Can you go over Cardio?

Question: What is an Ejection Fraction? Can I code
an Ejection Fraction of 0% as with occlusion?

Epileptic Seizure Coding
Question: Can I get opinions on the correct
diagnosis coding for seizure (in an epileptic patient)?
If final diagnosis code is seizure, but the patient also
has epilepsy, should you code to 780.39 or go to the
345.xx series of codes?

Question: Can you tell me if some of the leveling
exercise answers may be incorrect? E/M
It says to "drop the lowest" on the 2 of 3 Office
established patient so I ended up getting several
wrong when I did that ( ie. PPL, I put, 212,212,213)
and it said 212 would have been correct on the
answer sheet I printed out.... maybe I'm not getting
it?

between how coding is done for each side
(Inpatient - vs - Outpatient).

Sequencing
Can you address sequencing? I sometimes struggle and wonder if sequencing in the order of the
What, Why, how, where would be the correct direction to take in coding sequence.
Annual Medicare Visit
If you go for a annual medicare visit (not initial) and you ask the patient if there are any areas of
concern or problems and they list their stomach has been sore but they think it's due to
overexertion, can you bill for checking the stomach separately?
Certification for Document
Is there a certification just for documentation specialist,and is this a good transition or step up for
someone who currently does coding only?
