The document is an end user/end use certification for export controlled items from the United States. 1. END USE/END USER CERTIFICATION
Please read and certify the following export control notifications:
1) I (We) acknowledge that the items to be received from Richard Maxwell ., under our purchase
order(s) are possibly export controlled from the United States by the International Traffic in
Arms Regulation (ITAR) and may be listed under the United States Munitions List (See 22CFR
121).
2) I (We) acknowledge that items controlled under the United States Munitions List require the
registration of manufactures and exports with the U.S. Department of State, Directorate of
Defense Trade Controls (DDTC) prior to any export from the United States (See 22CFR 122).
3) I (We) acknowledge that items controlled under the United States Munitions List prior to export
or temporary import, unless the export or temporary import qualifies for an exemption under
the ITAR, require an export license or other approval from the DDTC prior to any export from
the United States (See 22CFR123).
4) I (We) acknowledge that items controlled under the United States Munitions List, before
exporting any hardware therein controlled, using a license or exemption, the DDTC registered
applicant/exporter, or an agent acting on filer’s behalf, must electronically file the export
information with U.S. Customs and Border Protection using the Automated Export System (AES)
in accordance with appropriate time lines (See 22CFR123.22).
5) I (We) acknowledge that items on the United States Munitions List exported from the United
States will comply with the requirements specified under U.S. regulations and also in compliance
and full knowledge of Items 1 through 4 listed above and the ITAR (See 22CFR Parts 120-130).
I (We) certify, as an authorized employee, knowledge of the above listed items and will assume full
responsibility for compliance with any United States export laws and regulations prior to any export
from the United States of product(s) received from Richard Maxwell
