1. • History
• c/o frequency, nocturia, poor stream,
intermittency x 3 months.
• No hematuria, lithuria/fever with rigor and
chills.
• No h/o any fractures.
• No h/o loss of weight.

2. • Past History: No h/o DM,HTN,CAD/COPD
/Seizures. No h/o drug allergies.
• No previous surgeries.
• Family history :
• No h/o any urological malignancies in the
family.

3. • On Examination :
• Pulse rate- 80/min, BP- 130/80 mm Hg
• P/A : Soft , non tender. LE: Meatus- normal
phallus- normal. B/L testes & cords : normal.
• DRE: grade II prostate, hard consistency, non
nodular, non tender.

4. CNS: Patient is conscious, co-operative and well
oriented to time, place and person.
- CVS: S1, S2 normal
- RS: AEBE

6. USG abdomen (04/08/22):
• Bilateral kidney normal in size, shape, with
maintained CMD, increased echotexture.
• Urinary bladder partially distended.
• Prostate- 28cc, heterogeneous parenchyma
with infiltration of bladder neck & apex.
• Multiple enlarged retroperitoneal and left iliac
lymphnodes.

7. MRI PROSTATE (03/03/23):
• Prostate- aperas bulky, volume measuring
3.3*3.8*3.8 cm involving PZ with extension to
CZ & TZ, superiorly into bladder neck,
posterolaterally ECE into NVB.
• Multiple enlarged lymph nodes in b/l internal
iliac, ext. & common iliac & para aortric region
• Mets. Infiltration to pelvic & lower
lumbosacral bones.

8. HPE (13/2/23) :
• Adenocarcinoma prostate , score 3+4=7
