Written permission is required for any additional adults to pick up a child from Lady Bug's Child Care. The adult must be at least 18 years old and the parent must provide their current address and two contact phone numbers. Once written permission is given, the parent only needs to verbally inform the daycare of the planned pick up date and time. However, this verbal permission must be provided before the scheduled pick up or else the child is legally required to remain in the daycare's custody until the parent picks them up.

1. Additional Adult Pick-up Form
Written permission MUST be given to Lady Bug’s Child Care in order
for any additional adults to pick up your child from care (Non-
custodial parents / Grandparents / Aunts / Uncles / Friends). This
adult must be 18 years of age or older and you must list a current
address and 2 phone numbers that the adult can be reached at in care of
an emergency. Once your written permission is given, you only need
to verbally give permission of what day and time the other adult will be
providing pick up for your child. The verbal permission MUST be given
to me before the pick up is to take place—otherwise, I am legally
required to have your child remain in my custody until you come to
pick him/her up.
